AFFIDAVIT OF DISCREPANCY

I, Joshua Edward L. Sanchez of legal age, single, with residence and

postal address at #74 Aurora Pijuan st. BF Resort, Las Piñas City,
Philippines, after having been duly sworn to in accordance with law, hereby
depose and state:

1. That in my Certificate of Live Birth as recorded

in the Office of the Civil Registrar of Las Piñas City, Philippines and
Tertiary Student Permanent Record at Mapua University, it is stated
that I was born on 01 Feb 1996 at Las Piñas City, Philippines;

2. That my relatives, acting on the honest belief that I was born at Las
Piñas City, had used Las Piñas City as my place of birth in all
pertinent school records when they processed my enrolment at
Mapua University in Intramuros, Manila;

3. That when I filled up my Civil Service Commission record for the

purpose of taking the Professional PPT exam required by the Civil
Service Commission last 13 Jan 2017, I had completely forgot to fill
up in the “Place of Birth” section and because of that, I had
unconsciously left it at blank instead of putting “Las Piñas City” in
that particular section;

4. That I am executing this Affidavit of Discrepancy to attest

to the truth of the foregoing facts and to explain the discrepancy in
my Certificates of Live Birth, Tertiary Student Permanent Record
Transcript of Records, and Professional Regulatory Commission ID
as regards to my place of birth;

Further, I am declaring that from this date hereon I would be using my

true and correct place of birth at Las Piñas City in all my papers, records and
other documents.
