    Survey Questionnaire for the Implementation of HRIS:


    Time and Attendance Module

    Objective: This tool aims to gather information and to develop a better understanding of the
    challenges/problems being experienced/faced by CSC users in the implementation of the
    HRIS.

    Direction: Please answer the following questions truthfully. Editable copy of the
    accomplished questionnaire shall be submitted to OSM through osm.csc@gmail.com on or
    before March 29, 2019.

    A. Policy Awareness - What do you know about the policy’s objectives?

    B. Content Relevance - What part of the policy do you consider positive? Negative? What is
    interesting about it?

    C. System Effectiveness - What do you think of the Time and Attendance Module of the HRIS?
    Is the system effective/ineffective relative to its objective? Please explain.

    D. Process Efficiency - How do feel about the procedures to implement the policy? Are the
    processes fast or slow? Efficient or inefficient? Could you share your experience?
