CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This Agreement made and entered into this ____ day of ____________ in ___________,
Philippines, by and between:

THE _______________, a local government unit duly organized and existing under
Philippine law with office address at _______________, hereinafter referred to as the FIRST
PARTY represented by ________________., as the Municipal Mayor of _______;

hereinafter referred to as the SECOND PARTY.

WITNESSETH:

WHEREAS, the First Party will celebrate its Patronal Town Fiesta,
“________________” this month of ______.

WHEREAS, the Second Party is an artist, comedian and a television host.

NOW THEREFORE, in consideration of the mutual covenants and agreements herein

contained, the parties are hereto, intending to be legally bound, agree as follows:

TERMS AND CONDITIONS:

1. The First Party engages the services of the Second Party to provide hosting service.
The service to be performed at Event Location:

2. The Second Party hereby agrees to provide a Hosting service for the First Party at the
above- mentioned location.
3. The Second Party hereby agrees to render his professional services at all times.
4. The Parties hereby agree that the Hosting Service shall be provided and accepted on
the following date and time of the engagement:

5. The First Party in consideration of the service to be rendered by the Second Party, and
the mutual promise contained herein, hereby agrees to pay the latter the following
consideration:

A performance fee amounting to AMOUNT IN WORDS [Php 00,000.00] Pesos

including all the taxes, travel, food, accommodation and other necessary expenses
A down payment fee of 50% amounting to AMOUNT IN WORDS [Php 00, 000.00]
shall be given to the Second Party to secure his services. This amount shall be applied
toward the Performance Fee.
The remaining AMOUNT IN WORDS [Php 00, 000.00] shall be given after the
event.

The First Party and the Second Party agree that this contract is not subject to cancellation unless
both parties have agreed to such cancellation writing.

That in case of breach of contract, case shall be filed exclusively at the Regional Trial Court of
__________.
