1. PURPOSE
Petrochina International (Jabung) Ltd. is a Production Sharing Contract
Contractor for SKK Migas as the operator for the Jabung Block. Petrochina
International (Jabung) Ltd. intends to install their pipes under the river to maintain
performance. This project is to install pipes from NEB Basement#1 To NEB# 87
using the Auger Boring method. All activities must consider the potential hazards to
be handled and be safe for personnel in the cargo.

2. SCOPE
This HSE Plan created to cover Safety, Health and Environment (HSE) related
activities performed by PT Ade Konstruksi Abadi (CONTRACTOR) for Pipe
Crossing Construction Services For NEB#87 and NEB Basement#1 of Petrochina
International Jabung Ltd. (COMPANY). Provide CONTRACTOR's planning to
ensure the requirements are fulfilled including HSE requirements concerned with
project execution, identify & assess project specific (known) hazards and document
where they are present, and ientify the procedures to be implemented to meet the
requirement.

3. DEFINITIONS AND ABBREVIATIONS


3.1. DEFINITIONS

COMPANY : PETROCHINA INTERNATIONAL (JABUNG) LTD.


CONTRACTOR : PT. ADE KONSTRUKSI ABADI
INCIDENT : Unplanned event or chain of event which has or could
have caused injury or damage to assets, reputation, third
parties or the environment.
ACCIDENT : Incident which resulted in injury or damage to assets,
reputation, third parties or the environment.

ENVIRONMENT : Accident which involves damage to the environment.


DAMAGE The impact on the environment is considered damage
when legal environment requirements or internationally
recognized standards have not been fulfilled. Excessive
discharge over a certain period is considered damage.

FIRST AID CASE : Operation related injury or illness which can be


treatment by first Aider, with a standard first aid kit, and
does not require a professional physician.
MEDICAL : Operation related injury or illness which does not
TREATMENT involve restricted nor lost workdays but requires
CASE treatment by a professional physician. A Medical
treatment case is the consequence of an injury.
LOSTTIME : Operation related injury which renders the injured
INJURY person unable to perform any regular or restricted work
on the day following the accident, including if this day
is a rest day, a vacation or public holiday/ weekend.

4. LEADERSHIP AND COMMITMENT


It is the CONTRACTOR obligations to ensure that adequate safety levels are
maintained, and environmental impacts are planned for and monitored during the
entire performance and perform of the WORK comply with the HSE requirement.
This HSE Plan describes the HSE rules that must be followed by all
CONTRACTOR workforces. Each of Personnel must ensure that they are working or
going to work in a reasonable safe place, and that their activities will create no risks
to himself and others, also to equipment and to assets. And for consistency and
compliance purpose, CONTRACTOR commits to follow and complies all the
COMPANY HSE rules that are applicable during the contract performance.
The CONTRACTOR Managements will monitor directly the compliance to
COMPANY HSE policy and HSE requirement and its implementation during
contract performance.

5. ORGANIZATION AND RESPONSIBILITIES


5.1. HSE ORGANIZATION

Responsibility and its relation to the implementation of HSE within


CONTRACTOR will be reflected in Project organization Chart as bellow:

The HSE role and responsibilities for each function are summaries as bellow:

5.1.1. HSE Manager


CONTRACTOR HSE manager is direct report to the CONTRACTOR
top management. His primary responsibilities are to ensure the HSE
program is well implemented during the whole project execution. The

HSE manager primary duties will include but not limited to the
following:
a. Assist CONTRACTOR Project Manager to prepare the Project HSE
plan specific for project activities and/or for reviewing the Project
HSE Plan submitted by Subcontractor, this may include discussion
and interpretation of COMPANY standard, regulation and rules set
down in the plan.
b. Review the HSE resources manpower and commit to efficient
personnel to perform the scope of work.
c. Liaise with CONTRACTOR Project Manager to monitor the
execution of the HSE activities at the design, construction,
installations and contract/project operational stages and ensure that
supervisory staff and the project personnel can demonstrate an
understanding of the HSE procedures and instructions with apply to
their particular responsibilities.
d. Liaise with CONTRACTOR Project Manager to define and monitor
the performance of HSE ultimate goal as defined in Project HSE
Objective or HSE key performance indicator.

5.1.2. HSE Coordinator


Shall report to the Project Manager and is responsible for directing and
verifying the effectiveness of HSE systems and control verification
during the project execution. The Project HSE Manager is to be
responsible for safety activities and environmental planning and
implementing of safety standard procedures for all project sites.
a. Initiate HSE awareness within the project team, reports & update the
progress status of all HSE corrective actions;
b. Organize project HSE audit program, compile audit reports for
submission to the PMT and COMPANY;
c. Preparation of HSE Plans, Programs and Procedures;
d. Review and comment on CONTRACTOR’s project procedural
documents addressing the control and verification methods which
may affect HSE conformance to specification requirements;
e. Development and execute audits of the Project HSE Management
Plan;
f. Coordination of Task Risk Assessments;

g. Liaison with COMPANY operational staff with respect to safety


standards, permits etc.;
h. Interfacing with COMPANY Representative and COMPANY HSE
Manager on project related HSE matters through the
CONTRACTOR Project Manager;
i. Reporting daily on all safety and environmental aspects of all site
activities; and
j. Provide support and Assistance to the Project Manager and
Emergency Response Team to manage accident, emergency and
crisis affecting the project.

5.1.3. HSE Officer


HSE Officer on site is assigned as the assistant of HSE Coordinator in the
case of HSE. HSE officer has the authority of the HSE Coordinator to
assume responsibility as described above. In addition, in cases where the
project location or field work does not meet the minimum HSE criteria and
has the potential to create a risky situation, the safety officer in the field can
stop work in the field. With responsibilities as bellow:
a. Supporting Project Manager in carrying out its responsibility to
implement HSE plan in the field
b. Advise HSE applicable rules, guidelines, rules and workplace standards.
c. Ensure that the HSE Plan requirements are consistent with the
requirements of all workers and CONTRACTORS in the field.
d. Assist in preparing and improving HSE Plan as part of continuous
improvement process.
e. Coordinate workplace inspection programs and regularly conduct HSE
internal audits both systems, and / or processes, procedures and work
practices.
f. Assist in identifying, assessing and developing hazard control
mechanisms in the workplace.
g. Assist in investigating and reporting an incident at work.
h. Ensure that HSE information is systematically arranged and distributed
to all workers.
i. Coordinate, collect, and record HSE database.

j. Maintain value information on legislation, industry work rules and other


relevant standards.
k. Attend and coordinate safety meeting and safety talk in the field.
l. Prepare and conduct safety induction to all workers involved.
m. When an emergency occurs, the safety officer acts as a supporter and
liaison to all parties concerned.
n. Always provide examples of safe and healthy action as role models to all
workers.
o. Supports in developing Job Safety Analysis

6. PROJECT ORGANIZATION
This section describes the overall HSE and Project Management structure ang
organization in order to execute this project.
Responsibility and its relation to the implementation of HSE within
CONTRACTOR will be reflected in Project organization Chart as detail in
attachment 1 - HSE and Project Organization Chart. And the HSE role and
responsibilities for each function are summaries as bellow:
1. Project Manager
The Project Manager will report to Director. The Project Manager is to
manage the entire effort for execution of the Work. Project Manager has the
authority to take any action required to maintain a satisfactory project
performance across the project organization, such as the Engineering,
Procurement, Onshore Fabrication and Installation, Pre-commissioning,
QA/ QC and HSE Teams. The Project Manager is responsible for the overall
management of the project, including interfacing with CONTRACTOR and
COMPANY’s. Specific responsibilities include:

a. Lead by example and remains abreast of all Project HSE requirements;


b. Ensure Project HSE Plan and Project Waste Management Plan are
prepared, reviewed, and submitted to COMPANY for approval. And
ensure these plans are implemented throughout project;
c. Initiating Contract review by collation and dissemination of contract
documents to the Project Management Team; responding to queries and
comments and ensuring that all specified requirements of the

COMPANY obligations including the Safety and Quality requirements


are fully understood and satisfactorily completed;
d. Ensuring that efficient and effective communication is maintained both
within the CONTRACTOR’s organization and with COMPANY to
expedite the provision of the Services and demonstrate compliance with
the Contract;
e. Planning, organizing and directing the activities of all the Disciplines in
the Project team;
f. Ensuring that design of products with performance including
workmanship and productivity meets the HSE and Quality
requirements;
2. QA/QC Manager
a. Ensure deployment of workforces and Subcontractors comply with HSE
Policy requirements and restrictions;
b. Ensuring the development and implementation of a project specific
Quality and HSE Procedures;
c. Ensuring until the assistance of the QA/QC Co-ordinates that controls
are in place to prevent the delivery of nonconforming products or
services until the deficiency or nonconformity is rectified;
d. Ensuring that corrective and preventive actions are implemented to
maintain the continuous improvement in Quality performance;
e. Ensuring that the resources are adequate and qualified to perform the
Quality related activities. Implement and arranges all necessary training
for the personnel to perform the quality activities;
f. Where required, liaising with the COMPANY and Certifying Authority
on matters relating to quality, certification, inspection etc.;
g. Interfacing with COMPANY Representatives and COMPANY QA/QC
Manager on project related QA/QC matters through the
CONTRACTOR’s Project Manager;
h. Ensuring all quality issues regarding the work is resolved in an
acceptable and timely manner;
i. Arranging and coordinating surveillance inspection of procured
materials, as appropriate;
j. Ensuring that systems and procedures etc. are in place for quality
activities in all functional areas;
k. Ensuring compliance to COMPANY Specification;
l. Monitoring quality issues and inspection activities on site(s);

3. Coordinator Engineering
a. Ensure deployment of workforces and Subcontractors comply with HSE
Policy requirements and restrictions;
b. Technical interface & check for the integrity of the Discipline
Engineering Teams;
c. Management of the Discipline Teams;
d. Performance of the Discipline Teams;
e. Quality assurance within the Engineering Discipline deliverables;
f. Review and coordinate towards approval of engineering deliverables;
g. Environmental impact on Design;
h. Health, Safety, Security and Environmental consideration in all
Designs;
i. Interfacing with COMPANY Representatives on project engineering
related matters;
j. Interfacing, establishing and controlling all Subcontractors’ designs,
regularizing Technical Query (TQ) / Site Instructions through drawing
revisions, reviewing Subcontractor’s shop drawings, etc.;
k. Interface & liaise with the Procurement Team and Construction Team
regarding design issues;
l. In conjunction with the Project QA/QC Manager ensuring Company
requirements and all technical aspects are addressed in the Inspection
and Test Plans;
m. Ensuring all design issues within available materials, transport
requirements, regulations and constructability are carefully considered;
n. Manage engineering man-hours, cost, change order requests;
o. Monitoring Engineering Subcontractors to ensure that all engineering
documents are delivered as per schedule; and
p. Interface with the Fabrication & Installation teams to provide
engineering support.

4. Project Supervisor
Project Supervisor means all position acts as the team leader for the
team who will execute the job. It is including the project coordinator and
engineers. The main task of him consists of the following:

a. Conduct induction sessions with the Project team and site on the use of
the plan.
b. Monitor personnel mobilization, demobilization including visitor on the
project locations.
c. Liaise with CONTRACTOR Construction personnel to monitor the
execution of the HSE activities at construction, installation and

contract/project operational stages and ensure that supervisory staff and


project personnel can demonstrate an understanding of the HSE
procedures and instructions which apply to their responsibilities.

5. Procurement Manager
The Procurement Manager reports to the Project Manager and is
responsible for all procurement activities on the project, including
procurement weekly reports and expediting progress status. The
Procurement Manager shall be responsibilities for:

a. Ensure deployment of workforces and Subcontractors comply with HSE


Policy requirements and restrictions;
b. Implementation and management of procurement activities
b. Liaison and expediting with the vendors and suppliers.
c. Involvement in all bidding, negotiations on Specific Terms &
Conditions and Final Purchase Order issuance;
d. Supervision of all procurement operations and personnel to ensure all
project requirements are monitored and progress achieved;
e. Follow up with logistics coordinator for master list preparation &
approval, clearing imported materials from customs etc.;
f. Preparation of specific Project Procurement Plan and Procedures,
Materials Management Procedures, Subcontracting Procedures
inclusive of a procurement organization chart to support and provide
details to the overall project organization Chart.
g. Submission Expediting Report on Weekly basis.

7. HSE POLICY AND STRATEGIC OBJECTIVE


1. HSE POLICY
The HSE Policy is declared and endorsed by CONTRACTOR Director and his
management, and widely communicated to all employees through all
communications means. It policy is to ensure that everybody will be aware and
understand with our common task and set of principles, goals and requirements;
for the implementation and maintenance of HSE regulation, especially for
CONTRACTOR Management of the project to fully satisfy the COMPANY in
respect of HSE requirements, contractual scope of work, standards and
specification and legal requirements, and to fully support the CONTRACTOR
Project Team to fulfil the obligations on time with the highest safety and quality

standard. The CONTRACTOR HSE policy is state in attachment 2-Contractor


HSE Policy.
2. STRATEGIC OBJECTIVE
Prior to the project execution the CONTRACTOR set up specific HSE
objective and target in line with the COMPANY HSE objective. The HSE policy
and objective will be communicate to all employees during internal HSE
induction and to ensure awareness among all employees related HSE policy and
objective, CONTRACTOR will develop kind of communication program such as
by HSE alert, notice board, meeting, mail, etc.
CONTRACTOR Goals are “zero accident” no personal injury or illness, no
property damages, and no pollution to the environment during performing all of
the project activities. Refer to HSE Program and Key Performance Indicator
(see attachment 3), these goals will be strictly monitored trough HSE key
performance Indicator, as bellow:

8. POTENTIAL HAZARD AND RISK MANAGEMENT


The Risk Assessment is based on the analysis of levels of occurrence probability
and possible severity. To quality and quantity the involved risks of an activity,
CONTRACTOR utilizes a Risk Assessment Approach. The Risk Matrix also allows
determining the reduction of the risk obtained after the establishment of control
measures, application of working methods and/or use of Personal or Collective
Protective Equipment.
The reduction of risks will follow the rule of ALARP (As Low As Reasonably
Practicable). To reduce a risk to a level which is as low as reasonably practicable
involves balancing reduction in risk against the time, trouble, difficulty and cost of
achieving it. This level represents the point, objectively assessed, at which the time,
trouble, difficulty and cost or further reduction measures become unreasonably
disproportionate to the additional risk reduction obtained.
The evaluation is done for each hazard associated to activities taking into account
that one hazard may involve different consequences and different levels of risks.
The procedure implemented for the hazard Identification and Risk Assessment is
refer to COMPANY Risk matrix. And risk register will describe in JPSC-230-41-
PCD-4-103-00-HIRADC.

9. PLANNING AND PROCEDURE


The CONTRACTOR and project team will determine operation and activities that
are associated with the identified hazards where the implementation of controls is
necessary to manage the occupational health and safety risks and shall include the
management of change if required.

1. Permit to Work
All persons who will involve in this project are required to use and understand
Company Permit to Work system prior activities, contractors must have trained
permit to work system. After attending the permit holder and/or work party.
The permit shall be used to align with requirement company PTW procedures. all
jobs with potential risk shall be completed with a work permit as part of
communication between permit holder/work party and the person who issuing
permit, permit authority. permits are submitted for approval a minimum 24 hours in
advance for the job that will be carried out to enable permit authority to review and
assess the associates risk of respected job prior to approved and issue the permit. the
PTW system tasks that will use in this project will be:
A. Hot work permit
B. Cold Work Permit
C. Excavation Work Permit
2. Job Safety Analysis
Refer to contractor job safety analysis procedure, job safety analysis (JSA) is an
approach designed to identify potential hazards in employment and plan corrective
actions. JSA focuses on the safety of every potential activities. And shall be submitted
prior ptw released. Please See Attachment 4 For Procedure And JSAForm

3. Housekeeping
CONTRACTORS will implement activity routines for maintaining good
housekeeping standards and material storage at the worksite. Housekeeping program
will be performed daily before permit closed. CONTRACTORS will maintain all
storage areas and materials in accordance with good housekeeping practice fit to
CONTRACTOR Housekeeping Procedure. All storage areas must have adequate
illumination and ventilation. Material is not stored or placed in a manner that restrict
or block access or traffic aisle. Escape route for emergency situation, walk way and
traffic aisles must be clearly marked and provided with illumination. Hazardous
substances shall be stored in secure areas and in accordance with Material Safety Data
Sheets / manufacturer’s recommendation. Please see attachment-5 for Housekeeping
Procedure.

4. Motor vehicle safety


Road traffic on construction area shall be strictly limited to the vehicles dedicated
to the project related works only. Private cars and other vehicles shall not enter the
construction area.
Legal traffic safety rules such as fastening the seat belt and enforcement of speed
limits shall be strictly applied within the WORKSHOP and public road. Vehicle
conditions shall be inspected regularly to minimize injuries and reduce risks to
personnel.

5. Electrical Safety
Installation of ELCB (Electrical Leakage Circuit Breaker) on electrical hand tools
at WORKSITE to be implemented. Electrical equipment must be suitably earthed,
where it cannot be earthed, it must be double insulated.All energized electrical
equipment / panels / switchboards / cables shall inspect by competent person and
complete with inspection sticker.
Only authorized personnel (electrical maintenance personnel) are permitted to
operate machinery, to start and operate electrically driven equipment and to energize
or de-energize electrical circuits or switchboards.

6. Material Handling Andstorage


Materials handling is a technique which includes the art of lifting, placing, storing
or moving of materials through the use of appropriate handling equipment and
personnel. Classification of materials handling operation is:
1. Manual Handling
Lifting, transporting and packaging of products using own physical strength
and hand operated. All personnel shall be more vigilant due to some incident
have occurred during performed work like bending back, twisting with load,
load too heavy, reaching too far, lifting to one side, off-balance shifting and
failure to use PPE.
 Precautions in manual handling:
 Familiarize with the hazard associated with the material or load like sharp
edges, weight of material, material content (hazardous or non-hazardous)
and check MSDS.

 Examine the surrounding area prior to handling


 Get a firm grip of the object
 Use necessary personal protective equipment
 Lift gradually
 Follow proper lifting method

2. Mechanical Handling
Mechanical Handling Operations are those activities which involve the
movement of a load using work equipment. This work equipment can be that
which is either manually operated or power operated. When a Mechanical
Handling Operation requires a load to be raised or lowered then the required
work equipment becomes lifting equipment and the activity can be considered
to be a lifting operation. Examples of mechanical handling equipment which is
not lifting equipment would be a wheeled trolley used to move a load.
However, if such equipment is used to move a load up or down an incline then
this would become a lifting operation.
Mechanical handling equipment could be categorized into:
 Mechanical equipment offers more power. However, more power often
times means more hazards and more severe injuries. The following pre-
cautions should be observed when working with some mechanical handling
equipment:
 Cranes: only thoroughly trained and competent persons are permitted to
operate cranes. Operators should know what they are lifting and what it
weighs.
 Slings: when working with slings, personnel must ensure that they are
visually inspected before use and during operation. Riggers or other
knowledgeable employees should conduct or assist in the inspection
because they are aware of how the sling is used and what makes a sling
unserviceable. A damaged or defective sling must be removed from
service.
General requirements for material handling:
 Operators must be authorized
 Equipment must be regularly inspected and maintained
 Never stand underneath loads or leave loads or leave loads flying above
your head.
 Give warning or instructed to keep away to other employees whenever they
are in danger because a load in passing over the heads.

 Use a hooked rope to replace a hanger from pipe.


 Stand at a piece where a load of a pipe is flying away from you and not
approaching you.
 Never stand between loads any object at rest.
 Report every broken or damage hanger to your supervisor.

7. Barricades Tape
Barricades are commonly used and abused in the work area. Although usually put
in place for good reason, they are often disregarded. Planning the use of barricades,
maintaining the barricade once in place, enforcing observance of the barricade and
removing the barricade when the work is complete are all essential elements of
effective use of barricades.

8. Welding
The welding process joins metal parts used 2 (two) techniques, is Shielded metal
arc welding (SMAW) and Gas tungsten arc welding (GTAW). The gas welding
process unites metals by heating. The gases commonly used as the fuel gas are oxygen
and acetylene. The gas cutting process removes metal by a chemical reaction of the
base metal with oxygen at an elevated temperature.
The arc welding and cutting process use electric current and two welding leads.
One welding lead is connected to the electric power supply while the other lead is
attached to the work surface. Additionally, safety hazards can exist such as the
potential for fire or explosion and injuries from arc radiation, electrical shock, or
materials handling.
Refer to Contractor document JPSC-230-31-PCD-4-104-00 Welding book for
Pipeline, these provides guidelines for safely performing welding operations. It
presents provisions for training, discussion on types of welding, safe work practices,
and employee protection requirements. It also presents critical details on hot work
permits, works in confined spaces, ventilation requirements when performing welding
operations, and inspection requirements.
SMAW welding, like other welding methods, can be a dangerous and unsafe
practice if proper precautions are not taken. The process uses an open arc, which raises

the risk of burns. So, a welder shall be using a complete PPE, besides the standard
PPE heavy leather gloves, a long sleeve jacket and face shield to protect them.
Tungsten Gas Arc Welding (GTAW), also known as inert gas tungsten welding
(TIG), is an arc welding process that uses a non-consumable tungsten electrode to
produce welds. The weld area and electrode are protected from oxidation or other
atmospheric contamination by inert protective gases (argon or helium), and the metals
used normally, some welds, known as autogenous welds, do not need them. The
constant current welding power supply produces a fully charged, electric energy arc
through a highly ionized column of gas and metal known as plasma.
And to reduce flashbacks in the welding process, the flashback arrestor must be
installed and done before it is done. All welding equipment used must be inspected
before use and welding test by Company team. And all precautions will assess and be
written on job safety analysis.

9. Personnel, Equipment Certification and Operational Control


CONTRACTOR ensure controls the operational related tools, equipments and
services. CONTRACTOR also controls the Subcontractor regardi ng personnel and
equipments Personnel shall competent and follow mandatory and special training.
Personnel before mobilize to site should have certificate stamp FIT to work from
COMPANY medical. The equipment is required to monitor or measure of it’s
performance. CONTRACTOR will manage, inspection, maintenance also calibrate the
equipment’s. Records of these activities will be kept by CONTRACTOR.

10. Communication
CONTRACTOR line management and supervision are responsible for
communicating HSE information and procedures to all personnel involved.
CONTRACTOR acknowledges the importance of effective communication procedure
the hazards, effective means to communicate HSE Issues to the COMPANY,
CONTRACTOR and Subcontractor. The purpose is to inform and socialize hazard,
program, standard, etc. This action plan can be implemented by means of:

10. WASTE MANAGEMENT AND DISPOSAL


The impacts on the environment of the COMPANY’s activities on this project
shall be assessed. Waste shall be collected in segregated place as the COMPANY
standard requirement and disposed regularly. The system shall be communicated in the
site induction or as a toolbox talk.
The work area shall be maintained to a high standard of cleanliness, and
supervisors will be expected to review and rectify their respective work areas as
required on a daily basis.
Waste material shall not be thrown into water, soil, or air. Where there are specific
recycling bins, material is to be segregated and disposed of in the appropriate bin.
Waste will collect at the container (bin). General waste will transport to final disposal.

11. MEDICAL REQUIREMENTS


CONTRACTOR ensures fitness of employees to work in specific jobs or working
environment. CONTRACTOR will conduct pre employees Medical Check Up to
examine the fitness of personnel. This will assist management to assign personnel who
has good fitness to work before going to specific work site.
CONTRACTOR also has annual Medical Check Up program for employees. No
employee or project personnel can work or come to site unless have current approved
medical certificate. Medical Check Up must be conducted in accordance with the
guidelines laid out in COMPANY site and conducted by COMPANY recommended
MCU Providers. Medical certificates will be reported to COMPANY for review and
approval. COMPANY medical team will issude Fit to Work MCU for personel who
qulified fit to do the jobs.

12. PERSONAL PROTECTIVE EQUIPMENT


All persons entering the Project site shall be required to wear the following PPE
where designated
- Wearpack

- Safety Helmets
- Safety Shoes

- Safety Glasses
And also additional PPE required depend on the jobs, include but not limited to
- Safety Hand Gloves
- Safety Harness

- Ear Muff and Ear Plug

- Respirator
- Rubber safety boot

13. ACCIDENT/ INCIDENT INVESTIGATION AND REPORTING


This procedure is used as a guide in conducting reporting activities and
investigating incidents. Includes stages in the event of injury / injury, fatality or near
miss or all findings of non-conformity and form of treatment and stages in the accident
investigation.
All incidents occurring in the field are reported as soon as possible to the site
manager, supervisor, and HSE Officer will notify the incident in notification and
reported form to the project management team and company. As below incident

Investigations conducted by the investigation team involving contractor HSE
officer, supervisor and site manager. Prior the investigation revealed are done, the
results of the investigation should be reviewed at the appropriate management level
(through daily, weekly, monthly or special meetings) that will check the completeness
and quality of the investigation and to justify the recommended action.
The results of its investigations Accidents are discussed in project coordination
meeting, management review meetings and are used to maximize the outcomes of
learning and assist in achieving the objectives of preventing the recurrence of similar

accidents. And the results of the investigation are documented using an accident
investigation reporting form. Please see attachment 6 for incident notification and
investigation form.

14. EMERGENCY RESPONSE PLAN


CONTRACTOR will develop specific ERP and contingency plan to
accommodate the maintenance activity on site to response to all identified emergency
situation. CONTRACTOR commitment and goal state that the objective of the
emergency evacuation is to minimize the consequences of acute illness or injury
occurring at the work place. CONTRACTOR will provide all necessary training of
firefighting and first aid to undertake their duties in a safe manner. CONTRACTOR
will provide Clinic Puskesmas Sukarejo before send IP to Hospitalif there is a work
accident.CONTRACTOR will establish an emergency team and organization which
will be posted on the site wide notice boards, and updated, as required, by change of
personnel. Please see document of Emergency Response Procedure on JPSC-230-41-
PCD-4-102-00 ERP.

15. MANAGEMENT OF CHANGE


Work arising from temporary and permanent changes to organization, personnel,
systems, process, procedures, equipment, products, materials or substances, and laws
and regulations could not be proceed unless a Management of Change process is
completed where applicable to include
a. A risk assessment conducted by all impacted by the change
b. Development of a work plan that clearly specifies the time scale for the
change and any control measures to be implemented regarding:
o Equipment, facility, and process
o Operations, maintenance, inspection procedures
o Training, personnel, and communication
o Documentation
o Authorization of the work plan by the responsible persons through
completion
