OF COUNSEL FOR CAUSE

The Plaintiff’s counsel de officio who undersigns this motion,

most respectfully states that:

1. He has already terminated his attorney-client relationship with the

Plaintiff particularly because of ___________________;

3. In view of the foregoing reason(s), undersigned counsel has found

it difficult if not impossible to fulfill his obligations as lawyer to
the plaintiff and continue with his cause; and

4. Undersigned counsel most respectfully requests that he be

allowed by this Honorable Court to withdraw his appearance in
this case as counsel for the Plaintiff with the latters’ express
conformity.

PRAYER

Considering the foregoing, undersigned counsel most

respectfully prays that they be allowed to withdraw his appearance
in this case as counsel for the Plaintiff and that he be relieved of all
their responsibilities relative to this case.
