citizens and a resident of Bayambang, Infanta, Pangasinan, after having sworn to in accordance
with law hereby depose and say:

That, we are the parents of MARK JAYSON M. DIMALANTA, 19 years old, and a resident
of Bayambang, Infanta, Pangasinan;

That, we are not employed to any government or private agencies, not engaged to any
gainful occupation, have low income and belongs to the indigent families in our barangay and
exempted in paying taxes;
