AFFIDAVIT of FACT

I, RONEL LARAYA ALCANTA, of legal age, and with residence address at

Brgy. Maraykit, San Juan, Batangas, after being sworn in accordance with law do
hereby depose and state that:

1. I used to work as FOOD SERVICE CREW at Al Maousherji

Catering Company (McDonald’s Kuwait) from December 31,
2004 to December 28, 2007. Proof of which is a Certification
hereto attached as ANNEX “A” and made an integral part
hereof;

2. Further, I had successfully completed the Crew Chief Course

given last November 4-5, 2007 at McDonald’s Kuwait. Proof of
which is a Certification hereto attached as ANNEX “B” and
made an integral part hereof;

3. Furthermore, I was consistently chosen as Employee of the

Month in 2005, 2006 and 2007 by the Management of McDonald’s
Kuwait. Proof of which is a Certification hereto attached as
ANNEX “C”,”D” and “E” and made an integral part hereof;

4. I am executing this Affidavit to attest to the truth of the

foregoing and for whatever legal purpose it may serve.
