1) Findings:

In accordance with ACI 350.1-10, clause 2.3.1.1 and Contractor’s Procedure “Water Leak Test Procedure for
Underground Pits – 424/20/00/CS/PC/NA/009381 Rev B1” concrete surfaces shall be visually inspected and
repaired, if any, prior to filling it with water.

In accordance with approved Inspection and Test Plan 424/20/00/QA/PC/NA/00924 Rev 00, Contractor is required
to perform formal inspection and notify Company to collectively agree that the subject water containtment
structure (TAG # 59-A8753, 59-A8754, 59-A8755) is fit for leak testing.

RFI # 424-QK-C-0852, 424-QK-C-0854, 424-QK-C-0856 were raised for preparation of leak test of Oily Water Pits TAG
# 59-A8753, 59-A8754, 59-A8755. But these pits were found to be filled with water before the inspection has taken
place. Further the following actions were not completed prior to filling the water for leak test

 Ladder rungs were not installed for all the three pits
 Surveillance report (622-SR-014) issued on Oily Water Pit TAG # 59-A8753 & 59-A8755 is still open

This is a deviation from international standard and project procedures. Construction team is advised and
indentify the gaps in the execution process and rectify immediately.
