Dr. I am so sorry for not informing you about patient JUDY ANN DE GUZMAN
on and off restlessness this morning. And I was not able to send her for ct scan. I
was thinking of referring it to you but due to flooding of admissions ,and I am
alone in the ward, this already emptied my mind. I admit it’s my fault not telling
you dr. I am aware too that is in my duties and responsibilities to tell or report it
to you dr. but this already had taken place yet I am assuring you this will not
happen again.
