Test Recovery Plan

Dear student,
If you got a grade that is undesirable on a test, I strongly recommend you spend time reviewing the unit and
proving your understanding on the concepts that you misses (and grade!). If you plan to take me up on this
opportunity, here are the requirements:

A. Write a summary stating what concepts were hard for you and what you believe you got wrong.
B. Student get a signature from parent/guardian to be made aware of test recovery plan.
C. Meet with Dr. Carson and look at your summary, and then look at your test to determine what areas you
need to do more research. Meeting date is scheduled for: _________________________
D. Come up with an action plan to prove student understands the concepts that they got wrong on the test.
Final product should be hand written- can be a poster outlining the concept with real-world examples,
paper, etc. Determined by student and teacher together.
E. Product MUST be turned in no later than 1 week after test is returned. Date ______________________
