OIC Regional Director
Government Center Candahug, Palo, Leyte

Attention: MR. DEAN ENDRIANO, Regional SPED Supervisor

Dear Sir/ Madam:

Warmest Greetings from Department of Health

Our office in partnership with the Eye and Hearing Foundation International (EFHI), Resources for the
Blind Inc. (RBI), Edmund Rice Ministries (ERM) and CBM, will be having an Inclusive Health project in the
region. This is to strengthen policies and systems for an inclusive, comprehensive, quality eye healthcare
including low vision and Hearing services to become accessible for the marginalized group and people
with disabilities in the target areas. These eye and ear care services include regular clinical visual and
hearing assessments, eye surgery and cerumen extraction and rehabilitation among others. For this
reason, we will be implementing said program which aims also to link children with visual and hearing
impairment to clinician such as ophthalmologist, optometrist, EENT Doctors and therapist for immediate
and appropriate medical attention.

In line with this, our office plans to hold an eye and ear screening specifically to the identified schools
with Sped programs and other selected schools in the region. Hence, we would like to ask permission in
your good office to allow our Prevention of Blindness Program Regional Coordinator Mrs. Winnie C.
Dorego and the Inclusive Eye Health Team to visit these schools and conduct the activity for S.Y. 2019-
2020. Attached is the list of schools and schedule of visit for your reference.

Many thanks for partnering with us and in considering our request.

Sincerely,

Dr. Ma. Sol Villones
