AFFIDAVIT OF DISCREPANCY

I, CAROLINA BERTIZ VALENZUELA, of legal age, Filipino and a

resident of Purok Ilang-ilang A, Poblacion Uno, Real, Quezon after having
been duly sworn to in accordance with law depose and say:

That JULITO L. VALENZUELA is my husband, who was born on April

23, 1952 in Infanta, Quezon;

That in our Marriage Contract, his name was mistakenly stated as

JEOLITO VALENZUELA, instead of the correct name JULITO VALENZUELA
as stated in his Certificate of Birth and Senior Citizen ID;

In all his other documents, he is using his correct name JULITO

VALENZUELA;

That the names JEOLITO VALENZUELA in our Marriage Contract and

JULITO VALENZUELA in all his other documents refer to one and the same
person who is my husband;

And that I am executing this Affidavit to attest to the truth of the

foregoing statement.
