RE: NOTICE OF TERMINATION

Ms. _______________,

Through this letter, we regret to inform you that your employment with
________________is TERMINATED, EFFECTIVE IMMEDIATELY.

Your termination was brought about by your non-payment of:

(1) Business’ laundry bill with Wash and Dry in the amount of SIXTEEN THOUSAND
TWO HUNDRED THIRTY FIVE PESOS and TWENTY CENTAVOS (P 16,235.20);
and
(2) Tourism Tax in the amount of THREE THOUSAND SEVEN HUNDRED PESOS (P
3,700.00),

despite the release of funds to you, as evidenced by signed cash vouchers.

The immediate return of the amount above stated during the service of the Notice to Explain on
29 May 2019 to you only serves as an admission on your part that you embezzled or
misappropriated said funds.

The reply you submitted in relation to the Notice to Explain does not justify the deeds you have
committed.

Your acts constitute serious disobedience and fraud or willful breach of the trust reposed in you.

Therefore, after consideration of all the evidences present, the management have decided to
terminate your employment.
