 Item 311 - Wooden instead of steel forms were noted used on the PCCP which does not
conform to the requirements at Sta. 0+000 – Sta. 0+314.
 Item 311 - No grooves/keys were noted on the longitudinal edges of the on-going
concrete pavement at Sta. 0+000 – Sta. 0+314.
 Item 506 - Insufficient mortar grout was noted on the spaces between boulders of the
completed section of the stone masonry structure at Sta. 0+133 to Sta. 0+148 and Sta.
0+180 to Sta.0+201, both at right side.
 Item 506 - Non-provision of weepholes on the stone masonry slope protection was
noted at Sta. 0+000 to Sta. 0+314, right side.
 Item 506 - Underrsized stones were noted incorporated in the constructed/completed
stone masonry structure at Sta. 0+180 to Sta. 0+201.5 and Sta. 0+224 to Sta. 0+230,
both at right side.

2. P002-52-5-55LZ – Construction of Multi-Purpose Building Construction of

Multi-Purpose Building Brgy. Caculangan, San Juan, La Union

 Item 403 - Insufficient splice length of installed sway brace was noted at the
constructed multi-purpose building.
 Item 403 - Incorrect orientation of angular cleats was noted on the purlin to truss
connections of the constructed building
 Item 311 - Temperature cracks were noted on the cement plaster finish at all columns
of the constructed multi-purpose building.

3. P002-52-5-54LZ - Construction of Multi-Purpose Building - Construction of

Multi-purpose Building, Brgy. Sto. Rosario, San Juan, La Union

 Sir Ressie eto yung tubular na MPB. Baka gusto niyo Sir palitan po ito Sir kasi Sir eto
yung iba ang truss design doon sa plano tsaka sa welds palang Sir spot weld na hindi
full weld. Baka gusto niyo po Sir palitan.

4. P002-00-8-59LZ - Construction of Road Slope Protection Structure - San Juan

- San Gabriel Road - K0280 + 093 - K0280 + 400

 Item 511 - Bulging of the gabion cages was noted at Sta. 280+260 to Sta. 280+380.
 Item 511 - Cross-connecting wire was not provided on gabion at Sta. 280+260 to Sta.
280+380.
 Item 511 - No selvedge wire was provided on the exposed ends of the basket joints at
Sta. 280+300 to Sta. 280+400.
  Item 511 - Oversized/undersized rocks were used in gabions/mattresses at Sta.
280+160 to Sta 280+400
 Item 511 - Non-provision of filter cloth as per plans and specifications at Sta. 280+160
to Sta 280+400.

5. P002-01-5-70LZ - Off-Carriageway Improvement: Shoulder Paving /

Construction - San Juan - San Gabriel Road - K0282 + 000 - K0282 + 482

 Item 311 - Asphalt sealant instead of pressurized concrete epoxy was noted as sealing
material for the transverse crack of the concrete pavement at block nos. 24, 29, 92, 96
and 106, all at left side.
 Item 311 - Transverse crack located within the middle third of the block was noted on
the completed concrete pavement at block no. 67 right side.
 Item 311 - Transverse crack within 1.50 m from the weakened plane joint was noted on
the completed concrete pavement at block no. 6 right side, and at block nos. 33 and 54,
both at left side.

7. P002-52-5-52LZ - Construction of Multi-Purpose Building - Construction of

Multi-Purpose Building, Brgy. Canaoay, San Fernando City, La Union

 Item 403 - Non-provision of nuts and washers were noted at at GL B/1-6.

 Item 900 - Insufficient levelling grout between the column and base plate was noted on
the constructed building at GL B/1-6.
 Item 900 - Bulging of structural concrete was noted on beams of the on-going
construction of the building at GL B/1-6.

8. P002-52-5-46LZ - Construction of Multi-Purpose Building - Construction of

Multi-Purpose Building, Brgy. Namtutan, San Fernando City, La Union

 Item 403 - Non-provision of king post wast noted on the installed steel truss.
 Item 403 - Non-provision of diagonal web member connecting the column and roof truss
was noted at GL 1.
 Item 900 - Honeycombs were noted at roof beam GL B/3-4.
 Item 900 - Insufficient levelling grout between the column and base plate was noted on
the constructed building at GL B/7.
 Item 1100 - Exposed electrical conduit was noted at GL A/6 and GL B/6.
