The applicant has applied for admission to the Children's Ministries Institute® and has requested that you
give a recommendation.

A personal recommendation gives insight into a person that would be very helpful in determining the
person’s ability to handle the long hours and heavy workload in the classroom as well as handling
relationships outside of the classroom. Please be candid and objective.

There are three ways you can submit this reference form.

1. Email - Fill out the form and email it to Admissions following the instructions below. (Note: When
you type your name into the signature line, it serves as an electronic signature.)

a. Before filling out the form, save the attached file as “CMI Reference Form student’s name”
on your computer. (ex - “CMI Reference Form Sarah Brown”)
b. Open the file in Microsoft Word and fill it out.
c. Save the changes.
d. Then attach this new file and email it to Admissions at cmiapplications@cefonline.com with
a subject line of “CMI Reference Form.”

2. Fax - Print the form and fill it out. Fax the completed form to Admissions at 636-456-4435.

3. Mail - Print the form and fill it out. Mail the completed form to
