AFFIDAVIT OF DISCREPANCY

I, JOHN MARK PABLO, of legal age, Filipino, married, and a resident of

Camaal, Quirino, Isabela, after having been duly sworn in accordance with
law, do hereby depose and state that:

1. I am the same person executing this Instrument;

2. I am executing this Instrument to certify that I am married to MARIA

PABLO on April 26, 2018 at Camaal, Quirino, Isabela which was
registered in the Office of the Local Civil Registrar of Quirino, Isabela;

3. Since then, I started to change my status from Single to Married and

used it in all my transactions, papers, documents, and identification
cards;

4. However, when I secure a copy of my Voter’s Certification with the

Office of the Election Officer in the Municipality of Quirino, Isabela,
my status reflecting was “Single” instead of “Married”;

5. I am executing this Instrument to certify that the person whose name

is MARK PABLO with status “Single” in the said Voter’s Certification
and the person whose name is JOHN MARK PABLO in all my
pertinent papers, documents and Identification Cards refer to one
and the same person, herein affiant;

6. I am executing this Instrument to declare and attest to the veracity

and truthfulness of the foregoing statements and for whatever legal
purposes it may serve.
