Register the Client ‘The immunization register, patient chart and immunization card provide a record of health services delivered at the clinic. They also help to identify missed opportunities for vaccination. 1) When a client arrives at a health centre or outreach site the first thing you should do is register him or her. Ensure that you have all the information necessary to fill out the immunization register, and to locate the client in the future if necessary: © Child's name © Child’s date and place of birth * Mother's name, locating address, and telephone number What is a locating address? Ifa mother fails to bring in her child for the next scheduled immunization, the CHA will need a specific address or phone number where the mother can be contacted. A locating address is an address which specifically identifies where a person lives together with the use of local landmarks local landmarks. Assess the Client ach client's health status and immunization status should be assessed prior to immunization, “Assessment of health status is discussed in the Family Health Manual, and usually consists of @ routine history and physical examination at well-baby, family planning, or curative visits (refer to Family Health Manual) The following questions should be part of the assessment of immunization status.
