PANGASINAN CHAPTER 2nd QUARTER MEETING

(Minutes of the Meeting)

I. CALL TO ORDER

The meeting was called to order at 10:00 PM by the PYAP Pangasinan Chapter President, Jemuel
P. Consuelo of Anda, Pangasinan.

The meeting commenced with an Opening Prayer, followed by the singing of the Philippine
National Anthem and the Pangasinan Hymn.

The secretariat then acknowledged the attendees of the meeting.

II. Attendees:

Said meeting was attended by twenty-five (25) participants which includes the Youth Focal Persons
& Municipal PYAP Presidents from the Municipalities of:
Anda
Alcala
Aguilar
Bolinao
Basista
San Fabian
Asingan
Lingayen

III. Review and Approval of the Agenda:

The following are the proposed agenda:

1. Presentation and submission of Quarterly Accomplishment Report
2. Immersion Outreach Program (IOP)
3. Assumption of Office re: Regional PYAP President
4. Other matters:
a. Provincial Youth Camp
b. PYAP Pangasinan Chapter Institutional Visit
c. 3rd Quarter Meeting Schedule and Venue

The presented agenda then was approved by the body.

II. Meeting Proper

AGENDA REMARKS
1. Presentation and submission of Only the Municipality of Anda has
Quarterly Accomplishment presented and submitted the said
Report report. It was agreed by the body
that the other Municipalities will
submit their reports through e-mail.
Thereafter, the Provincial Focal
Person on Youth Welfare, Ms.
Florence N. Berba also presented the

PSWDO’s accomplishments in line
with the Youth Welfare.
At present, there are eight (8) IOPs
assigned in the following
Municipalities:
- Anda (2)
- Aguilar (1)
- Basista (1)
- Asingan (1)
- Alcala (2)
- Villasis (1)
The IOPs shall submit the following
requirements after their 30-day
immersion:
- Evaluation Form
- Daily Time Record
- Accomplishment Report
It was agreed by the body that the
said requirements will be submitted
either personally or through mail to
Ms. Iryn Cubangbang of DSWD Field
Office I, San Fernando City, La
Union.
The presider, Jemuel Consuelo
updated the body on the sudden
death of the newly-elected Regional
PYAP, Mr. Jhun C. Sumera who died
last April 6, 2019 due to vehicular
accident. As per the constitution and
by-laws of the said association, in
case of death of the President, the
Vice President shall take the
presidency. Hence, as the elected
Vice President of Regional PYAP,
Jemuel Consuelo with the guidance
of Provincial Focal Person on Youth
Welfare shall coordinate with the
DSWD FO1 re: the
procedures/requirements in his
assumption of the position.
a. Provincial Youth Camp
For the 2nd Quarter activity, the
PYAP Pangasinan Chapter will
conduct its 2-day Provincial Youth
Camp on May 30-31, 2019 which will
be held at the Hotel 45, Baguio City.
Once the proposal and the program
are finalized, the body will be
informed through messenger group
chat.
 b. PYAP Pangasinan Chapter
Institutional Visit
For the 3rd Quarter activity, the PYAP
Pangasinan Chapter will have its
institutional visit at the Regional
Rehabilitation Center for Youth,
Magalang, Pampanga. The final date
and other information of the said
activity will be finalized on the 3rd
Quarter Meeting.

The Presiding Officer summarized the agreements and extended gratitude to all participants.

To formally close the meeting, Ms. Florence N. Berba gave her closing remarks to all the attendees.

The meeting was adjourned at 4:00 pm.
