This has reference of the failure to file on time of our leave application last December 18,2018
The delayed filing was due to inadvertence and mistaken belief that the application was already
filed when in fact, there was none. Upon discovery, I immediately caused the filing of the same
although it is very much delayed.

Hence, this representation is mot respectfully asking for a consideration to accept and approve
the aforementioned request with the promise that I will not commit the same mistake again.
