1. Term Duration of Employment. You shall be employed for a period of one (1) year commencing on __________________ to
__________________.Your employment contract is co-terminus with the service agreement of our client.

2. Compensation. You shall be paid a rate of P_____________ per day. The cut-off is every SUNDAY of the
month. The salary will be received every SATURDAY of the following week of the month.

3. Work Description. Your duties and responsibilities shall be in accordance to the job description made
known to you. You shall perform additional work as may require by the company from time to time under
the terms and conditions and according to the directions, instruction and control of the company.

4. You are expected to abide by the company rules and regulations of the company pertaining to job
performance, discipline, attendance and observe punctuality. Your official time schedule depends on your
Supervisor’s discretion. Should you violate any provisions of these conditions similar to abuse of discretion
amounting to lack of jurisdiction and destruction of the trust repose to you, the same shall have the right to
cancel the contract and finally terminate the services.

5. Any misrepresentation, regarding your previous employment, qualification, educational attainment,


personality or character, and or/or any medical findings, which will pose a threat or safety of other
employees shall be a ground for termination from employment.

6. You agree that all records and document of the company and its clients and all information pertaining to
their affairs are deemed confidential and at any time during or after your employment you will not make
unauthorized disclosure and reproduction of the same.

7. You further agree, that all records, documents and properties of the company or its clients in your custody
shall be immediately surrendered to the company, if requested during the employment period, and at the
termination thereof, whether or not requested.
EMPLOYEE BENEFITS
The worker shall, in addition to the salary/wage given to the worker, the benefit such as those given under the
Social Security System, Phil health, Pag-ibig and other such benefit that the Employer may voluntary grant subject
to the worker compliance with certain requirements in order to be entitled to the said benefit.
RIGHT TO TERMINATE THE CONTRACT
The company may terminate an employment contract for any of the following causes:
a. If worker is not competent or suitable for the above position;
b. If the worker fails to qualify in accordance with employers employment standards and
requirements made known to the worker at the time of his/her hiring;
c. If the worker violated any rules and regulations which the worker should know and study immediately upon hiring;
For any of the just causes recognized by the existing laws.

OTHER CONDITIONS

The worker agree, during the entire period of his/her employment to be transferred or assigned to any work station or position for such
periods as may be determined by the employer when the exigency or his/her service requires such transfer.

1. You also understand that in the event of involvement in any accident that will result to damage to any part of areas of your driven
vehicle or anything that entrusted to you by the Company, you shall be responsible for the cost of damage, regardless of whether
you are at fault or not. However, damage to the rear part of the vehicle or anything entrusted to you that can be attributed to you
will be charged accordingly.

2. In case you decide to resign from the company, you shall serve the company a written notice of 30 days prior to the affectivity date
of your resignation to give ample time for your replacement.

3. You agree that for and during the entire term of your employment, any and all kinds of information or data pertain to the company,
any of its partners, affiliates, or any of their clients including but not limited to customers data. Account information, internal
system, procedures and processes and other similar information obtained through or as a result of your employment, shall be
considered confidential and shall be kept as a private and privileged record of the company.

I hereby declare that I have read this document and fully understand before signing it and made it willingly and voluntarily and with full
knowledge of my rights under the law.
