RENEWAL CHECKLIST FOR REGISTERED NURSES & LICENSED PRACTICAL

NURSE

1.______ Application. Must be completely filled out and the affidavit portion on the
back must be notarized.

2.______ Pictures. Two (2) 2”x2” photos (signed on the bottom front portion of
each photo.)

3.______ Renewal fee - $100.00

Reinstatement fee - $150.00

4.______ Continuing Education. Thirty (30) contact hours are required to renew
your RN license. Sixty (60) contact hours are required to reinstate your
license. CE providers will be accepted from all Nursing Boards as well as
accredited sources.

All renewals are due one month / 30 days prior to expiration date of license.

If renewal application is received after the one month deadline, renewal will be
assessed a $100 late penalty fee; thereby total fee will be $120. Applications
received after the expiration date will be considered reinstatement and will need to pay
for the $150 reinstatement fee and must have completed sixty (60) contact hours (CE).

Only U.S cashier’s check , U. S. Postal Money Order will be acceptable forms of
Payment. Payments should be made payable to “NMI BON” or ‘NMI BOARD OF
NURSING”.
