documents from your good office:
 Certificate of Membership in the Philippine Bar;
 Official Bar Rating of the undersigned;
 Certificate of No Pending Administrative Case.

In this connection, I hereby authorize Atty. Nomela M. Cana to act as my

representative and to do the following acts for and in my behalf:

 apply, process, and pay for the issuance of the aforementioned

documents;
 collect said documents at your end for me;
 sign documents and papers for and in my behalf;
 execute any and all acts necessary for the issuance and release of the
aforementioned documents.

I also confirm that I shall be fully and solely responsible for the nondelivery of
said documents from my representative. Thank you in advance for your
