AFFIDAVIT OF DECLARATION & CONSENT

I, CHRISTOPHER G. HAWAN, of legal age, Filipino, Married and with address at MACTAN LAPU
LAPU, CEBU CITY, having been duly sworn according, to law, hereby depose and say:

1. That I applied for overseas employment as DIVER with Omanfil International

Manpower Development Corporation which I know to be a company duly licensed Land
Based Agency.

2. That I was recruited and hired by my employer HYUNDAI ENGINEERING &

CONSTRUCTION CO. LTD of said agency to work as DIVER in KUWAIT.

3. That I was informed by said agency that my entry visa will be under the category of
ELECTRICIAN issued by the Ministry of Foreign Affairs/Immigration Department of
KUWAIT.

4. That I consented to use the category of ELECTRICIAN as my ENTRY VISA considering

that I will be doing the job which I was recruited and hired for with the said employer,
subject to the agreed salary which was offer to me.
