Dear Sir:
I am Edward Vladimir P. Moncay, BSN-4th year, graduating student (March 2019) here
at COC-PHINMA. I humbly request your consideration and approval to be enrolled on subject
“SSA-001 Society and Culture with Family Planning and Population” 3 units-Tuesday and
Thursday 9:00 AM-10:30 AM, Room PH 307.

Sir, the reason, I was late in enrolling was that, I had been given permission to cross-
enroll at Capitol University because Sociology with Anthropology was not offered in our school,
and because I was a graduating student, I was allowed by Dean Gutierrez and the Registrar. I was
officially enrolled at Capitol University on November 9, 2018, College of Nursing and
Midwifery.

Unfortunately, my subject at Capitol University with subject title SOCSCI 6 Sociology

with Anthropology was DISSOLVED on November 26, 2018 due to lack of number of students.
We were only three(3) in the class.

When I visited COC PHINMA yesterday November 27, 2018 to confer my problems, the
Registrar informed me that the subject SSA001 Society and Culture with Family Planning and
Population 3 units, has been offered and I can re-enroll immediately. I readily asked permission
from Dean Gutierrez to drop in Capitol University and that same day went back to CU and
understandably, Dean Fidela Ansale of CU (College of Nursing and Midwifery) logically agreed
for my dropping out.
