AFFIDAVIT OF CONSENT

Marketview, Lucena City, after having been duly sworn to in accordance with law hereby depose
and state:

That I am the owner of a house and lot with an area of SIXTY ONE
SQUARE METERS AND FORTY SQUARE DECIMETERS (61.40 sq.m.)
which is part of Transfer Certificate of Title No. T-49447;

That I hereby give my authorization and consent to my nephew, ROCKY

VON SOLIDAS to use the said property in the above mentioned address as his
shop for free for his business, named “ROCKSICA CELLPHONE &
ACCESSORIES”; and

That I am executing this affidavit to attest to the truth of the foregoing

facts and for whatever legal purposes this may serve.
