AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ROLINE P. JULATON and ELEANOR D. ALALAG the undersigned,

both of legal age, married, Filipino, and presently residing at Cawagayan, Pinukpuk,
Kalinga, after having been sworn to in accordance with law do hereby deposes and
stated;

That we personally know Ms. Yvonne B. Bongabong who graduated at

Cwagayan National High School last School Year 2012;

That Ms. Yvonne B. Bongabong, has not involved in any school

demonstrations/trouble nor member of any left leaning group/criminal gang and no
derogatory records filed against her during the period.

That we executed this affidavit to state on record that Ms. Yvonne B.

Bongabong is a good person, Student of good moral character and declare the truth of
the foregoing and for whatever legal purpose it may serve.
