AFFIDAVIT OF UNDERTAKING

I, CHADE B. MATA, of legal age, Filipino, single and a resident of Buenavista,

Bohol, after being sworn to in accordance with law, depose and say that:

1. I am a nurse by profession, employed in the Nurse Development Project of

Department of Health assigned at Buenavista, Bohol under a Contract of
Service;

2. As such, we are required by the Bureau of Internal Revenue to duly file

and/or process our 1901 BIR Registration;

3. By reason that my contract of service is still on process, I am constrained to

defer the aforementioned BIR registration;

4. Nonetheless, pursuant to the preceding statements, I undertake, in good faith,

to comply with the said BIR registration requirement on or before August 2,
2018;

5. I am executing this affidavit to establish the veracity of the foregoing facts

afore-stated and for all other legal purposes this may serve best.
