AFFIDAVIT OF FIRST DONATION

I, DELIA J. ROMUALDO, of legal age, Filipino, married and resident of Brgy. Balabag,
Pavia, Iloilo, after having been duly sworn to in accordance to law hereby depose and say:

1. That I am the Attorney-in-Fact of GREGORIO J. JAMOLIN, JR. who is the registered

owner of Lot No. 410, covered by TCT No. 090-2014002215, containing an area of
ONE THOUSAND FORTY-NINE (1,049) SQ.M. more or less, and located at Brgy.
Balabag, Pavia, Iloilo;

2. That the portion of the above-mentioned landholding which are Sub Lot No. 410-A
with an area of TWO HUNDRED THIRTY TWO (232) SQ.M. and Sub Lot No. 410-E
with an area of TWO HUNDRED THIRTY THREE (233) SQ.M., both of Psd-06-084568;
are being donated to his niece, KARLENE JULES J. ROMUALDO by virtue of the Deed
of Donation;

3. That the said donation is the first donation done by the above-stated owner on his
properties this year 2015;

4. That this Affidavit is executed as a requirement for the issuance of Certificate

Authorizing Registration with the Bureau of Internal Revenue (BIR).
