Job Role Summary

The Urgent Care Lead Nurse will provide senior clinical leadership for the development and delivery
of the Trust’s strategy for urgent care services, working closely with the service Directors to ensure
the delivery of better outcomes for patients. This will include supporting the Integrated Urgent
Care, 999 and PTS services and also leading service developments as appropriate across the Trust. In
addition the Urgent Care Lead Nurse will provide operational guidance and support for the
development and implementation of new urgent care initiatives, including mental health, frailty,
advanced care planning, frequent callers, advanced practice and clinical advisory service
developments.
The post holder will provide guidance and clinical leadership to clinicians and managers regarding
urgent care delivery and related service developments including quality, care pathways, patient
involvement and best value for money. The post holder will work closely with the clinical directorate
and pathways advisors ensuring alignment in operational and clinical developments.

The post holder will provide nursing professional leadership in the Directorate, supporting the
development of the nursing workforce and maintenance of high professional standards of practice.

As a senior nurse in the organisation, the post holder will actively contribute to Trust-wide nursing
professional developments, working closely with the Deputy Director of Nursing and Quality and
other senior nurses.

The post holder to provide expert advice and support for Quality and Safety within the directorate,
PTS and IUC including oversight of compliance with CQC standards, systems for learning and
improvement and quality of patients’ experience.

The role will entail co-ordinating the contribution of other clinicians, clinical and operational leads,
and working with a wide range of stakeholders including primary care, community services,
commissioners and other urgent care providers, ensuring alignment of YAS developments to wider
health and social care system improvements.

Core Responsibilities

Communication and Relationship Responsibilities


 To ensure the Director for Urgent Care and Integration and other senior managers are kept fully
informed of all relevant issues related to Urgent Care.
 Report on service development and performance related issues, which may involve
presentations to large groups of stakeholders.
 Maintain open and robust lines of communication between the senior managers within the
service, ensuring that they are informed of operational or clinical issues arising so that they can
cascade all relevant information to their staff.
 Represent the organisation:
o within the local healthcare economy, actively fostering and developing working
relationships with other healthcare professionals, promoting the vision of service
delivery and to continuously improve and learn from best practice and to promote a
positive organisational image.
o at national and regional forums in order to develop a network of contacts and
knowledge to support the delivery of service and to promote the directorate and trust in
the health economy.
o locally, including acting as media liaison when appropriate, in relation to the services
and visions of the Urgent Care and Integration directorate.

Internal audit

Strategy, Policy and Service Developments

 To ensure alignment of YAS developments to the wider health and social care system
improvements.
 In collaboration with the Clinical Directorate support the development and implementation of
best practice days across YAS to lead a culture of innovation and change.
 To develop new and innovative ways of working in
alignment with the local health economy to provide a
consistent and safe access and assessment and entry
route for patients.
 To identify strategic business/service development
opportunities linked to urgent care through
engagement with stakeholders.
 Makes recommendations to appropriate groups within the Trust regarding implementations of
developments.
 To support the professional and personal development of directly reporting staff.
 To provide an expert contribution to support development of a patient focused urgent care
service within the organisation.
 To support and identify training required alongside the Associate Director of Paramedic Practice
for clinicians in delivering urgent care to enable improvements in patient care.
 Education and development of an open learning culture and staff engagement across the
organisation empowering and developing front line staff.
 To provide guidance and clinical leadership to clinicians and managers regarding urgent care
delivery and related service developments, including quality, care pathways, patient
involvement and best value for money.
 To lead the modernisation and development of the service, improving the quality of care,
leadership and development of staff by focusing on the clinical and technical aspects of the
patient pathway.
 In liaison with the Clinical Directorate:
o To ensure effective mechanisms are in place to audit clinical practice with Urgent Care
deliver a Continuous Quality Improvement programme for the directorate.
o To be responsible for the continuous monitoring and analysis of information against
quality standards, developing and implementing procedures to reduce clinical risk and to
improve clinical care in line with regulations, policies and procedures and national
guidelines.
o To be responsible for the monitoring and maintenance of clinical audits, i.e. use of
clinical pathways alongside the Clinical Directorate
 To review technical and operational procedures and policies in order to achieve and maintain
best practice across the directorate and implement change to ensure that long term aims are
achieved. To work with the Director for Urgent Care and Integration to review and develop
clinical policies and procedures in support of the directorate, reflecting the needs of patients at
all times, ensuring that the policies and practices operated within the Services and in line with
the wider Trust are clinically sound, safe and reflect those of the NHS strategic documents.
 To identify strategic business/service development opportunities linked to the directorate and
Trust through engagement with stakeholders.
 To understand what is happening in the Urgent Care arena, ensuring ongoing review with
commissioners to understand their needs and future requirements.
 To contribute to discussions with commissioners and other stakeholders to define the scope,
shape and delivery of future NHS Urgent and Emergency Care services.

Analytical and Judgemental Responsibilities


 Ensure any developments are patient focused and clinically safe.
 To clinically lead on the delivery of key developments
within the urgent care strategy, project managing the
achievement of specific improvement objectives,
with input from other Trust personnel and wider
stakeholders as required.
 Managing and supporting the developing of business
cases and financials associated with delivery of new models of care.
 Develop with other senior managers to implement an urgent care workforce in partnership,
ensuring provision of a modern workforce to meet changing health care needs and crossing
professional boundaries.
 Liaise closely with HR and staff side colleagues in the development of operational changes that
impact staff.
 To ensure a proactive and systematic approach to risk management in order that all risks
associated to Urgent and Emergency Care which have a potential adverse effect on the quality
of care, safety and wellbeing of patients, service users and the public are identified and that
appropriate actions and safe systems are implemented to eliminate or minimise the identified
risks.
 To actively manage risk by maintaining and updating a risk log and a lessons learnt log.
 To ensure that all incidents, accidents and serious untoward incidents are reported, investigated
and managed in line with Trust Policies and Procedures.
 To support the Trust’s incidents handling procedures by investigating incidents fully and
contributing to Clinical Incident Review and Serious Untoward Incident processes as required.
 To foster a fair and just culture where staff learn from incidents and are willing to report both
incidents and near misses.
 To be responsible for ensuring that robust procedures are in place within the directorate to
address any complaints in line with the Trust’s Complaints Policy, ensuring that any identified
changes are implemented and feedback is given to staff concerned.
 To be accountable for ensuring that evidence relating to the standards within CQC are
maintained for services within the directorate.
 To ensure that areas of activity relating to Infection Control, Child Protection and Security
Management are delivered in line with Trust policies and procedures, regional and national
guidance/requirements.
 To work within the trust standing financial instructions.
 Ensure optimum use of agency and bank staff within agreed financial targets providing safe
nursing standards within agreed financial parameters.
 Work closely with the finance team in implementing new models and understanding where cost
improvement plans can be made to improve quality.

Planning, and Organisational Responsibilities

 To work collaboratively and effectively in partnership with YAS directors and managers to deliver
the highest standards of patient care and experience, ensuring that clinical risk is minimised and
patients are assessed, advised and referred to the most appropriate health care facility for their
needs.
 To actively contribute to the successful team working
of the Trust Management Group to ensure that the
Trust achieves its corporate objectives including key
service targets.
 To highlight potential risks to delivery of patient care
and achievement of Trust objectives.
 To contribute to the development of corporate strategy, policy and decision making activity and
promote the mission, vision and values of the organisation.
 To contribute to the establishment and maintenance of good corporate governance
arrangements.
 To advise on recommended areas of internal audit focus in relation to urgent care.
 To represent the Trusts management group to create, review, monitor, and revise existing
policies in order to ensure and support the strategic development of the Trust for urgent care.

Research and Audit Responsibilities, Clinical Advice and Direction

 To identify the population at risk using local data and information from a variety of sources and
to make recommendations for the focus of current and future service development, audit and
research.
 To critically evaluate and interpret evidence-based research findings from diverse sources
making informed judgements about their implications for changing and / or developing services
and clinical practice. Using this to inform organisation clinical developments.
 To continually evaluate evidence and audit the quality and effectiveness of the practice of self
and others, selecting and applying a wide range of valid and reliable approaches and methods
that is appropriate to needs.
 Work closely with the universities to identify areas of improvement and courses that need to be
commissioned to deliver new services.
 In conjunction with the Medical Lead, provide clear and direct clinical advice to the Directorate
regarding clinical standards, appropriate treatment regimes and its statutory duties with regard
to clinical care.
 Support and advise clinical and other staff on clinical care issues and the resolution of clinical
problems and questions that arise following the provision of care, with escalation to the Medical
Director according to Trust policy.
 Advise and contribute to the development of new models of service delivery which increase
efficiency and lead to better health outcomes for patients, in line with current Department of
Health recommendations and Trust plans.
 To participate in the dissemination of clinical advice and direction that are applicable to the
directorate given from bodies such as NICE or JRCALC (as examples) and ensure local
implementation.
 Support and develop systems to provide clinical information to Trust staff to enable them to
benchmark and audit their practice so as to improve the patient experience ensuring safe and
reliable care

Leadership, Management, Training and Supervision Responsibilities

 Provide professional nursing leadership within the Urgent Care and Integration Directorate.
 Contribute to Trust professional nursing and clinical developments working closely with the
Deputy Director of Nursing and Quality, IUC Head of Nursing and other senior nurses within the
organisation.
 Provide professional leadership working closely with the Deputy Medical Director and the clinical
directorate in development of integrated urgent care.
 Ensure systems are in place to provide assurance on quality and safety in the Urgent Care and
Integration directorate, including compliance with CQC standards and delivery of CQUIN
priorities.
 Actively support engagement with patients and carers in service development and evaluation.
 In conjunction with the Director of Urgent Care and Integration, Medical Lead for Integrated
Urgent Care and the Clinical Directorate to provide strong clinical leadership at all levels
(strategic, tactical and operational) throughout the Trust.
 Improve practice and health outcomes so that they are consistent with national and
international standards through managing and facilitating change in ways which are effective in
their own context.
 Contribute to the wider development of their area of practice, through publishing and
disseminating developments in the interests of patients, clients and carers.
 Act as an expert resource by offering appropriate advice to their own and other professions on
care practices, delivery and service development.
 Challenge professional and organisational boundaries in the interests of patients, clients and
carers to improve care outcomes.
 To support the delivery of appropriate standards of care through clinical staff responsible for
delivering care in emergency, urgent and primary care settings.
 Act in a consultancy capacity regarding the therapeutic care of complex cases. This will include
taking the lead role in the multidisciplinary pre-hospital care team, including qualified
paramedics, emergency care support workers, emergency medical dispatchers and other groups
working either directly within the ambulance service or in roles supporting the ambulance
services task.
 At all times to place patient safety at the centre of Directorate activity.
 To participate in systems to monitor the delivery of clinical care across all areas of Trust activity
to ensure the safe and integrated care or treatment for patients across both health and social
care systems.
 Establish a working relationship with key stakeholders and external partners to ensure that the
needs of patients across the service are met including the liaison with other health care
providers, social care departments and commissioners to facilitate the management of patient
care across healthcare systems to maximise patient benefit.
o As a member of the Directorate Management team, positively contribute to the delivery
of the team’s objectives.
