AIM Process – Identification of Major Accident Hazards
The first task was to identify the major accident hazards. The major accident
hazards are those unplanned event with escalation potential for multiple fatalities
and/or serious damage to the asset and possibly beyond the asset itself. The same
was done by referring to various reports like Quantitative risk analysis, hazard and
operability studies, process and instrumentation diagrams, emergency
response plans and risk registers etc. The international database of
accidents for similar types of facilities was also referred. The identification of the
major accident hazards was done for each of the six installations.

Recommendations from Mining Breakout

1. Safety has to include not only the workplace


but all public and private sectors as well.
Therefore VISION ZERO should adress
kindergarten, school, university and private
life.

2. We should make clear that committment to


VISION ZERO is not a question of investment
and spending money for a project. Successful
prevention requires belief and attitude and in
fact will save money.

3. The balance between compliance oriented

should be made clear.

4. An important behaviour for managers and


workers for VISION ZERO is not to look away:

5. VISION ZERO is the responsibility of owners,


employers, managers and all workers. All
human beings in the enterprise must be
committed and involved. Families of workers
should be involved as well as they are
concerned.

6. Machinery suppliers have as well to be


involved in the VISION ZERO: OEMs deliver
too often incomplete machines – safety device
must be installed by customers or extra-paid.
Mining customers should speak with one
voice to the suppliers worldwide!

7. There is a need to further develop the


perception of RISK-ASSESSMENT from
bureaucratic burden to a mangement tool for
managers. This includes the use of short-time
risk management tools like « TAKE 5 ».

8. Especially in India the lack of compentence


and skills of the workers is a big safety issue
as well. Therefore it is recommended
 to develop the dual vocational training
 to intensify regular training of all
stakeholders at workplaces and to improve
the quality
 to introduce a obligatory training for
beginners

9. A VISION ZERO internet platform to share


knowledge, news, experiences is
recommended.

10.Though participants are convinced that India


should start a broad implementation of the
VISION ZERO now, there is uncertainty who
and how it could be done.

11.Bigger advanced companies should play an


important role to spread their safety culture to
SME´s and contractors. Subcontractors need a
specific focus. A specific safety-package for
this group is suggested.
5 General Remarks
1. The quality of the presentations was far above
average.
2. Some world class companies presented their
successful OSH-approach.
3. All speakers in Breakout B showed their
committment to the VISION ZERO – Strategy and
their enthusiasm.
4. The 7 Golden Rules were present in all
presentations – either directly or with regard to
contents.
5. Participants and speakers expressed their belief and
were convinced that VISION ZERO can become
reality.
