1. References:
a. Memorandum from C, WCPD dated July 1, 2019 with subject Absent
During Rank Inspection conducted by Field Evaluation and Validation
Team; and
b. HWI of DRDO to RCDS.

2. In connection with the above references, please be informed that the

undersigned was absent during the rank inspection conducted by Field Evaluation and
Validation Team on July 1, 2019 held in front of the PRO 12 Grandstand.

a. That the undersigned is the person in charge in making the slide

show presentation for the Weekly Staff Conference of the Office of
the Regional Personnel Records Management Division (ORPRMD)
that will be used every Monday;

b. That the undersigned was present on July 1, 2019 but not able to go
at the PRO 12 grandstand because before the rank inspection has
started the undersigned is making the slide show presentation for the
Weekly Staff Conference and the undersigned is present in the
formation in front of the admin building when the rank inspection
takes place.

3. Further, the undersigned fervently prays that the stated reasons shall
warrant merit from your good Office.

Detail of PCOs and PNCOs re Flag Raising and Friday Lowering.

2. In connection with the above references, please be informed that the

undersigned was absent during the Rank Inspection conducted by Field Evaluation
and Validation Team on July 1, 2019 held in front of the PRO 12 Grandstand at exactly
8:00AM.

c. That the undersigned is the person in charge in making the slide

show presentation for the Weekly Staff Conference of the Office of

1. References:
a. Memorandum from RESPO with subject: Personnel Who Were Absent
During Formation dated July 4, 2019;
b. Accounting of personnel conducted in front of the Admin Building, RHQ,
PRO 12, Tambler, General Santos City dated July 4, 2019; and
c. Verbal Instruction of Regional Directorial Staff during the conduct of
Daily Accounting of Personnel dated April 30, 2019.

2. In connection with the above references, please be informed that the

undersigned was absent during Formation dated July 4, 2019.

a. That the undersigned is the person in charge of the maintenance of

computers as IT PNCO of the Office of the Regional Personnel
Records Management Division (ORPRMD);

b. That the undersigned was present on July 4, 2019 but not able to go
in front of the PRO 12 Admin Building because before the Formation
has started the undersigned forgot to sign the attendance and the
undersigned is performing a reformat on the computers inside
RPRMD office that the undersigned cannot abandon the computer to
ensure its completion.

3. Further, the undersigned fervently prays that the stated reasons shall
warrant merit from your good Office.
