Belmont Asia Inspection Services Erbil, Iraq


QA/QC – 5/13/2013.
Visited Safar’s yard in Erbil to Monitor the Inspection process being conducted by Belmont Asia.
The equipment to be inspected: Mud Cross Spool w/ QTY (2) 11” 5000psi R54 Flange x QTY (2) 3
1/8” 5000psi flange.

Summary of discrepancies and actions:


On this Spool there was installed in the ring groove, a gasket R54. This was believed to be an
integral part of the spool by Belmont Asia inspectors and key personnel.
5/13: The effort to remove the “integral gasket” was fruitless by the team with the use of a
rubber mallet.
The Belmont Inspector hadn’t a metal hammer or chisel in their Tool Box.
 They were instructed to MPI a small area to Identify whether a flaw can be detected, which
when performed a flaw/detection was found. This would rule out the idea of this being
integral, also when the flange was cleaned properly the stamped information read 11”
5000psi R54.
 They were also instructed to perform a full body inspection. Then the spool will be sent to
the machine shop with the approval of Hi-tech manager (Kamal).

5/14: WZl QAQC Supervisor returned to Safar yard. The Spool was not inspected.
It was sent to ITS for removal of the gasket then to be inspected. ITS stated that they was unable
to hold the flange in the chuck (to large).
The Spool was sent to Pioneer Engineering. After completing the verification of Corpro equipment.
And a visit to ITS to work on the Spool, QAQC Supervisor was told that the Spool was sent to
Pioneer Engineering. Because ITS was unable to hold the flange in the chuck (to large).
 A visit to PE. Innospection Inspector and WZL-QAQC, endeavor to remove the gasket by
hammering on it. At the second strike, the Once Integral Gasket became divided. From the
groove.
 WZL QAQC Instructed IIS to perform a full MPI on the body, LP on the (1) 11” 5K and (2) 3
1/8” 5K ring groove with special Inlay and MPI on the (1) 11” 5K ring groove.
Inspection Result: (3) Ring groove size 11” 5K and (2) 3 1/8” 5K LP inspected OK/PASS. FIG 1, 1a.
