This template is to be used as a guide as part of the Commissioning Handover
Takeover process, it is not exhaustive and individual project requirements need
to be included.

summary with cross reference to functional design brief and final design

Task Matrix (a matrix listing the commissioning activities and the commissioning
team members and their roles in undertaking those activities).

Commissioning Issues Procedure (develop a procedure for recording and logging
issues as they arise and are resolved; the finalised log will be included in the
Commissioning Report).

Commissioning Forms (a listing of all the forms and documents to be completed for
each of the commissioning tasks for each of the disciplines and where they can be
obtained; they should all be appended to the Commissioning Plan; once completed,
these forms form the bulk of the Commissioning Report. Forms might include
task/action checklist, testing methodologies, test data, performance trend log, sign-off
procedures, issues log, etc. Completion of every form must include the signature of
the person(s) undertaking the work, date and the signature of all witnesses).

Liability Period including identification of responsibilities, output to Post Occupancy
Evaluations (POE) and recording documentation).

The Commissioning Plan may include the Defence Functional Design Brief, Final
Design Documentation and Basis of Design (if separate), Project Specifications,
Commissioning Meeting minutes, Commissioning Issues log, Construction
Checklists, Commissioning Forms, training materials, warranty listings, POE reports,
lessons learned.
The Commissioning Plan template should include the following outlines for typical
elements to be included in the commissioning requirements for a
building. The commissioning requirements should include, as
applicable, for each item of plant and system:

to demonstrate building performance in energy efficiency, water efficiency and
building environmental quality.

Commissioning Report

The commissioning report captures the detail and analysis from the commissioning
process and activities undertaken, namely outcomes from:
Factory tests and on-site (construction phase) tests of equipment and systems.
Full commissioning tasks at end of construction (as part of the handover /
takeover requirements).
Building tuning during defects liability.
The Commissioning Report should include all documentation recording that all
building systems are complete, tested and operational. Records of all testing and
balancing shall be provided, including factory test results, integrated system tests and
all unsuccessful tests with details on what was done to correct and finalise each item
(eg adjustment of set points, changes to control routines, details of faulty equipment).
It will provide information on the witnessing of tests and end of construction
commissioning, and involvement of NOD and CMSC during all commissioning
phases.
