NDP INDIVIDUAL AND TEAM SCORECARD

I. INDIVIDUAL EVALUATION
This tool is a modification of the existing NDP Scorecard. This shall be used to guide the Municipal/City Health Officer, Public Health Nurse and Development Management Officer in evaluating and
ranking the deployed nurses under the Nurse Deployment Project. Please take note that based on DOH Regional Memorandum Number 2017-0010, supervisors who are related with the NDP up to
the second degree of consanguinity and affinity are required to inhibit from participating in this evaluation process.

In filling up this form, kindly take note of the following instructions:


a. Each NDP Nurse shall be evaluated based on the specific functions/ activities indicated in Column 1. The evaluation shall include the NDP FHSIS Point Person who are currently handling at
least one barangay.
b. A point system shall be used to evaluate their level of performance, degree of accomplishment and professional behavior. The measures/indicators of performance are indicated in Column 2
while the corresponding points per measure/indicator are identified in Column 3.
c. The names of each NDP shall be printed in Column 4. Corresponding points per NDP per specific function/activity shall also be indicated in Column 4, under each NDPs name. Total points
per NDP shall be tallied in the last row. The highest possible number of points is 210 points
d. Each NDP shall then be ranked using the Summary Table of Individual Evaluation. The ranking shall follow a descending order, placing the NDP with the highest number of points in rank
number 1. The NDPs corresponding total score and adjectival rating shall be placed in the column provided.
e. The Municipal/City Health Officer, Public Health Nurse and Development Management Officer shall affix their signatures on the spaces provided.
f. This tool shall be submitted to DOH HHRDU not later than December 1, 2017.

III. TEAM EVALUATION


In conformance with DOH RO7 Regional Memorandum No. 2015-037, this form shall be used to assess the team performance of currently deployed NDP nurses. The evaluation process shall
exclude NDP FHSIS Point person who in turn shall completely accomplish this matrix. Accomplished tools shall be validated by the concerned Public Health Nurse, City/Municipal Health Officer
and Development Management Officer prior to sending. Assessment will be done per municipality and not per individual. Please refer to the official data contained in the Field Health Services
Information System. The fully-accomplished and signed matrix shall be sent to hhrdu.dohro7@gmail.com not later than December 1, 2017

Based on the individual and team evaluation, kindly appraise if the NDP Nurse is eligible for renewal. Please take note that according to DM 2017-0038 “Implementing Guidelines for the 2017
DOH Human Resources for Health (HRH) Deployment Program, only HRH with VERY SATISFACTORY PERFORMANCE are eligible to be rehired.
