1. The undersigned midshipmen would like to request from your good office an approval for
the request of Official Business Leave.

2. The purpose of the said leave is for claiming research sample results specifically lube
oil and diesel samples that were accumulated in their experiment for their Research
Subject (English 4) from SGS Philippines after conducting the Laboratory Testing.

3. If approved, the said leave will commence at 0300H Saturday, 17th of March 2018 and
will terminate on 2200H, Saturday, 17th of March 2018

4. SGS Philippines is located at 2229 Alegria Building Chino Roces Avenue, Makati

5. This is the list of the midshipmen whom are included to the said reporting.
