A meeting of the committee was held on 15/07/2018, at the registered office of the trust.

*UNANIMOUSLY RESOLVED that account in the name UMMID FOUNDATION to be opened

with CHOWPATTY Branch, HDFC Bank Ltd. And following Authorised Signatories be and are
hereby authorised to sign the documents to open and operate the account c/o UMER
HASSAN SHEIKH 5, FLOOR-GRD, PRAGJI RAGHAVJI CHAWL, SONAWALA AGIARY LANE, NR
CANOSSA H SCHOOL, MAHIM, MUMBAI - 400016 as per mode of operation and financial
powers mentioned below:

1. AYESHA RIYAZ SHAIKH

2. RAHIN RIYAZ SHAIKH
3. UMER HASSAN SHEIKH

Mode of Operation:

RESOLVED FURTHER THAT the said Bank be and is hereby authorised to accept, honour and
pass all Cheque’s, Hundi’s, Bill of Exchange, Promissory Notes, Indemnities’, Guarantee’s,
agreement for Letter of Credit, Trust Receipt for Monies, received and any other
commercial documents whatsoever drawn made, accepted endorsed executed as per
mode of operation mentioned above by the Authorised Signatories and to act upon all
instructions given by them relating to the account whether the funds in the account are
overdrawn or not, all of which shall be binding on the Trust.
