23rd FPSDC General Assembly Meeting

ADVISORY

A. TRANSPORTATION ARRANGEMENT

1. All participants are requested to purchase their travel tickets (plane, bus, boat etc.) as early as
possible to avail of discounted rates. Please check the schedule of activities so you can properly
arrange the time of your flights, land transportation etc.

2. All transportation costs such as plane, bus, boat, taxi shall be to the account of the participants.
However, FPSDC shall cover 80% of the Governing Board (GB)’s and 70% of the Committee’s
transportation cost (economy plane fare, boat fare and bus fare) in going to and from General
Santos. (To facilitate immediate reimbursement of plane/boat/bus fare, the GB and committee
member-participants are requested to bring with them a photocopy of their tickets and give them to
FPSDC staff (Sherry or Dulce) upon registration on May 4, 2019.)

3. FPSDC shall be providing transportation for the Lakbay-aral & Provincial Tour. However,
airport/pier transfers to and from the hotel will be to the account of the participants.
Vehicles arranged outside of what FPSDC will be providing shall be to the account of
participant.

B. MEALS AND ACCOMMODATION

1. FPSDC shall provide for all meals within the duration of the meeting. However, orders
outside of the meal package arranged by FPSDC shall be to the account of the participants.
Below is the schedule of arranged meal packages:

2. Accommodations at Greenleaf Hotel, General Santos City May 3-5, 2019 will be shouldered
by FPSDC. It should however be arranged ahead of time upon confirmation of the
participants. Failure to confirm will mean no reservation.

3. For those who wish to have an early arrival at Greenleaf Hotel on May 2 or would like to
extend their stay for another day or two, a minimal negotiated fee has been arranged by
FPSDC to be charged to the participant. It should however be arranged ahead of time upon
confirmation of the participants.

4. A full hotel and/or meal accommodation will be billed to organizations with more than one
participant.

5. Any incidental costs outside of what was arranged by FPSDC shall be individually settled
before check-out.

C. CANCELLATIONS

Any cancellation of reservations done before April 15, 2019 will be honored and no fee shall
be charged to the member organization. However, if cancellation is made from April 15
onwards, the member organization shall be billed the full amount of its reservation.

D. MEETING MATERIALS

The materials for the meeting will be electronically sent to your respective organizations
prior to the GA for your advanced reading. Please make sure that you bring your copies with
you during the GA.

E. OTHER ARRANGEMENTS

Other arrangements outside of what will be provided for by FPSDC must be communicated
with the FPSDC staff ahead of time.

To defray the cost of the assembly, a registration fee of P6,000.00 per official member
organization for the duration (May 3-5, 2019) of the GA shall be collected during the

registration period. However, for those who will be staying for another day or so at
Greenleaf Hotel, additional fee will be charged (subject to prior arrangement).

Guests / other participants who will be staying at the venue arranged by FPSDC shall be
charged with P5,000.00 per day (meals, accommodations, transportation for Lakbay Aral
and materials – P10,000.00 for two days). However, for those who will be staying for
another day at Waterfront Insular Hotel, additional fee will be charged (subject to prior
arrangement).

Live-out participants, if any, shall be charged with P6,000.00 registration fee to cover for
meals, transportation for Lakbay Aral and materials for two (2) days.

Other important details will be provided to all participants as soon as possible.

Thank you for your usual active participation. Kindly accomplish and return to
us the attached confirmation slip on or before March 22, 2019.
