Affidavit of Intent and Consent

I, GUINEVER D. MAÑACAP, ____ years of age, Filipino citizen, and

presently residing at Unit 14E North Tower, The Persimmon, M.J. Cuenco,
Mabolo, Cebu City, after having been duly sworn to in accordance with law,
do hereby depose and say:

1. That I was born out of wedlock to parents ARIEL R. MAÑACAP and

MARY ANN D. KONDO on 22 April 2005 in Cebu City;

2. That sometime in ______ my mother met Tomofumi Kondo (the

petitioner) whom she eventually marries on ____________________ at
___________________;

3. That since then my father ARIEL R. MAÑACAP allowed my mother to

have sole parental custody of me and let me live with her and Tomofumi
Kondo (the petitioner) as their child;

4. That Tomofumi Kondo desires to adopt me as his own child;

5. That Tomofumi Kondo has reared and cared for me even before he
got married to my mother and has been consistently providing me with
support for my basic necessities and education until the present;

6. That Tomofumi Kondo has developed a kind of parental love for me

as in fact he treated me as his true child;

7. That I love Tomofumi Kondo to be my father and he has in fact

treated me as his true child, and I hereby signify my written intent, freely
and voluntarily for Tomofumi Kondo to adopt me as his own child;

8. That I execute this affidavit to declare my intention and consent to

have me legally adopted by Tomofumi Kondo, as well as to the truth of the
foregoing facts.
