Memorandum of Agreement (MOA)

BETWEEN
DRA. CORAZON S. ARIOSA
AND

MS PROVIDER NAME: ZOOM TECHNICAL VOCATIONAL TRAINING AND ASSESSMENT CENTER, INC.
Enter into a Memorandum of Agreement with the DRA. CORAZON S. ARIOSA and the
hospital to provide an annual checkup of our students (depending on the number of
officially enrolled students) and personnel and equipment in response to a public
health emergency. This document is a binding contract as well as a guideline for operations.

I. Introduction: There is a recognizable need to coordinate in the event of our

annual checkup every 20th day of September.
This Agreement is for the provision of medical care services, and/or treatment.
II. Overview: It is agreed that MS Provider: ZOOM TECHNICAL VOCATIONAL
TRAINING AND ASSESSMENT CENTER, INC in Region IX
will provide medical support to the said HEALTH SERVICES.
DRA. CORAZON S ARIOSA and the Hospitals may provide appropriate training
for the administration necessary for the adequate protection and FIRST AID.

III. Goals of Medical Support Activity:

1. Medical treatment
2. Conduct check up
3. Enhance the working relationship through
Emergency Preparedness and Response community

2. Training: Offer training opportunities for MS service providers.

4. Data Collection: Provide guidelines, tools and technical assistance for data col
lection.
5. Training and qualifications are the ultimate responsibility of each MS Provider.
The principles and policies of this memorandum of agreement are hereby
understood and agreed upon with DRA. CORAZON S. ARIOSA and ZOOM TECHNICAL
VOCATIONAL TRAINING AND ASSESSMENT CENTER, INC. as Medical Service
provider indicated below.
This will amend any previous MOU/MOA and will remain in effect from the date of the signa
ture by both parties .
