ANNEXURE – I

Guideline for filling Annual Performance Appraisal Form

1. The annual appraisal form for the year 2012-13 shall be filled up by employees who has put in at least 6 months of service during
the period.
2. Note on Rating system:
2.1 The rating should be on a scale of 1-5 as follows

5 Excellent - clearly exceeds expectation
and achieved more than 3 times of the
Branch cost
4 Above Average – Exceeds some
expectations and achieved 2.26 – 3 times
of the Branch cost

3 Average – Meets expectations in general


and achieved 1.51 – 2.25 times of the
Branch cost
2 Below Average – Inconsistent
performance and achieved 1 to 1.5 times
of the Branch cost

1 Poor – Does not meet minimum


expectations and achieved less than 1
times of the Branch cost

2.2 On receipt of the self rating of the appraise, the appraiser shall have a discussion with appraise regarding his / her performance and self rating.

2.3 No decimal should be used in the rating. In case of weighted average rating has a decimal no, round it off to the next integer if it the decimal
no is more than 0.5 and the previous integer if it is less than 0.5.

2.4 The total weightage shall be 100%.The weighted average rating shall be calculated by Multiplying column (A ) with (B), (C) or (D) as the case
may be, add the products and divide by 100 and round off as indicated above.

2.5 The overall rating shall be calculated by giving 70% weightage for operational performance and 30% for Behavioral competency.
