CITY presently covered by TCT/CCT No. ___________ . I/We have attached copy/ies of my/our passport with
the corresponding entry and exit stamps for your reference.

I/We acknowledge and confirm that the aforesaid loan and collateral documents shall be notarized
after the certificate of title to the subject property has been transferred and registered in the name of the
undersigned principal and his/her spouse.
