INTRODUCTION
 The inspection was carried out in Larsen & Toubro Construction & Mining
Machinery Limited Bangalore, Karnataka. Construction and service of construction
equipments and mining machineries is done here.
 The inspection was carried out on 3rd July 2018. The inspection was carried out from
10 AM to 10:45 AM. The temperature was 30ºC in the industry.
 There were 3 work shift and around 800 people work for 8 hours in a single shift.
About 2400 people are currently working there. There are 4 entrance and 4 exit for the
company.
 The main purpose of the inspection is to identify the possible hazards and give the
suitable control measures to avoid the accident and to protect the workers. Also the
inspection was carried out to comply with the legal requirements and to avoid
financial and property damage.
 Work areas like assembly line, storage area, construction area, mining area, parking
area, service area, office, roadways for the vehicle, packing and shipping area, other
work departments, etc are covered in this inspection.
 Work equipments in the construction site, mining area, and service centre like jack
hammer, drilling machines, hammer, screwdriver, electric sander, spanner, chisel,
grinding machines, metal cutting saw, etc and other work equipments were inspected.
 Mechanical handling and mobile work equipment in the construction area, mining
area and service centre like conveyor, fork-lift, hoist, crane, backhoe loader,
bulldozer, crane, truck, road roller, tractor, etc were well inspected.
 The welfare facilities like drinking water, toilet facility, washing facility, transport,
resting and eating, first-aid, lockers, etc and work environment requirements like
space, seating, ventilation, light, temperature noise, emergency escape routes, etc
were also inspected.
 Several hazards were found while inspecting the industry, so I came up with the
suitable control measures to avoid the hazard before happening.

EXECUTIVE SUMMARY
During the inspection I found some major hazards which can be fixed easily and quickly in
order to avoid major accidents in the company. Fixing these hazards will improve the health
and safety in the company.
The men working at the heights of the construction area are using old expired safety harness.
In case of any accident these safety harness will not hold them. Provide them with good
quality and new safety harness.
Obstacles like broken wood, unused machines, damaged wires were found in the
3rd emergency escape route of the industry. Due to presence of obstacles the people mainly
the vulnerable group cannot use the 3rd escape route during fire accident and they will be
exposed to fire. Remove the obstacles and maintain good housekeeping in the escape routes.
I found vehicle travelling at high speed in warehouse which leads to collision with nearby
structure, people and stacking. Take strict action on the driver and only competent person
with valid driving licence must be used.
Also found the absence of fire alarm and smoke detectors in the mining machinery service
area. People may not know the fire hazard and people will get severe injuries due to fire.
Provide fire alarms and smoke detectors in the particular area as soon as possible.
The broken switches were found in the resting and eating areas, while using them they
produce shock to the user. Remove the broken switches and fix new and good quality
switches.
The backhoe loader was operated too close to the pedestrian pathway without any barricade
or signboards and the people were using the pathway. If the arm of the backhoe loader hits
the people it leads to severe injury, sometimes fatality. So change the pathway for pedestrians,
barricade the area and provide necessary sign boards.

MAIN FINDINGS
1) Falling Hazard And Recommendation

While inspecting the safety equipments, I found that the workers working at height of the
construction area are provided with old and expired safety harness. In case of any accidents
due to weather condition or slip and trip, these safety harness may fail to protect the workers
from falling from height. If the worker falls from height he may get serious injuries or fatality.

According to the section 7(A) of the Indian Factories Act 1948.Safety of the worker must be
ensured by installing and maintaining the work equipment in best possible safety conditions.
Tools, equipment, machines, or products used must be organized properly guaranteeing the
safety of workers.

Stop the work immediately or do the work with netting temporarily until the new safety
harness arrives. Buy new and good quality safety harness and provide it to the workers to
continue the job without any falling hazard.

2) Fire Hazard And Consequence

While inspecting the emergency escape route, the 3 rd escape route in the industry had obstacles
like broken wood, damaged wire unused machineries. In case of fire the people especially
vulnerable group will find it difficult to use the route. So they can’t escape the fire hazard in
correct time and it may lead to severe injuries, fatality.

According to the Section 11 of the 2005 Act. Employers are required to prepare and revise
adequate emergency plans and procedures and provide the necessary measures for fire fighting
and the evacuation of the workplace.
Recommendation and Cost Analysis

Remove the obstacles from the pathway. Provide proper housekeeping in the emergency
escape routes. Dispose the waste materials in the disposal area. Cost for new housekeeping
workers depends upon the salary packge.

3) Vehicle Hazard And Consequences

Driving the vehicle at high speed by the driver in the warehouse. Due to high speed the vehicle
may hit the nearby structure, stacks and people. If the vehicle hits the people it leads to serious
injuries and it may also lead to fatality.

According to the Section 112 of the Vehicle Act, 1988. No person shall drive a motor vehicle
or cause or allow a motor vehicle to be driven in any public place at a speed exceeding the
maximum speed.
Recommendation And Cost Analysis

Take serious action against the driver. Use only the competent driver with driving valid licence
holder. Provide necessary training, information and instruction.
Cost for hiring competent drivers depends upon the salary package.
4) Fire Hazard And Consequences

Absence of fire detectors and fire alarms at the mining machinery service area. In case of any
fire hazard the people inside the service area may not know the fire hazard and the may be
exposed to the file and get severe fire injuries and sometimes fatality.

According to the Section 11 of the 2005 Act. Employers are required to prepare and revise
adequate emergency plans and procedures and provide the necessary measures for fire fighting
and the evacuation of the workplace.

Workers should not carry any type of ignition sources to the area. Provide warning signs. Take
immediate action to fix fire detectors and fire alarms at the mining machine service area.
Cost of buying fire detectors and fire alarms - Rs 5000.

5) Electric Hazard And Consequences
While entering the resting and eating area, found the electric switches were broken and the
workers were using it. It provides shock for the worker who is using it. It may also lead to fire
hazard.

According to the Electricity at Work Regulations 1989. Require precautions to be taken against
the risk of death or personal injury from electricity in work activities.

Provide sign boards to avoid using the broken switch until fixing it. Remove the broken
switches and replace it with new and good quality switches so that the shock can be avoided.

6) Impact Hazard And Consequences

The backhoe loader was working so close to the pedestrian pathway. The barricade and sign
boards were not provided. If the arm of the backhoe loader hits the pedestrian then they will
get serious injuries, fracture, fatality.

According to the Section 21 of The Factories Act, 1948. Unless they are in such position or
of such construction as to be safe to every person employed in the factory as they would be if
they were securely fenced.

7) Entanglement Hazard And Consequence

Absence of protection guards in the coal conveyer working in the mining area and the workers
were working nearby. Loose cloths, loose hair, and other loose equipment can be pulled by the
rotating parts of the conveyers.

According to the section 21 of Indian factory act, 1948. Every rotating part off the machine
should be covered with the 360 degree guarding and fencing while working as well as not
working.

provide emergency shutdown switch to the conveyers.
Fix the necessary guards to the conveyers as soon as possible.

Conclusion
I thank the management for providing me such an opportunity to visit the company and allowing me
to do the health and safety inspection in the company. I hope my inspection will really help your
company to maintain the health and safety standards.
From my inspection I found that the welfare facilities drinking water, toilet facility, washing facility,
changing facility, transport facility, resting and eating facility, locker rooms and proper first-aid
requirements were properly provided and well maintained. The relationship between the worker and
the organization is good. This relationship helps in development of the industry.
Apart from this I also found some hazards in the work site and came up with proper control measures
to eliminate and reduce the hazards. There are few hazards which requires more attention and has to
be fixed as soon as possible. Failure to take action on these hazards will result in major accident.
Failure to fix these hazards may lead to loss of life, loss of company reputation, property damage and
to face the legal consequences.
For easy identification I have mention below the main findings with the effective recommend to
control the hazard:
1) Falling from height due to use of expired safety harness in the construction area.
2) Fire Hazard can cause due to presence of obstacles in the 3 rd escape route.
3) Vehicle Hazard can cause because of the over speed of the vehicle inside the warehouse.
4) Fire Hazard, Absence of smoke detectors and fire alarms mining machinery service area.
5) Electric Hazard due to broken switches and switch boards at eating and resting area.
6) Impact Hazard, Working with backhoe loader near pedestrian area at construction area.
7) Entanglement Hazard causes due to unguarded conveyers at the mining area.

implemented and actions are taken at the right time. And also take action on other hazards which are
mentioned in the observation and implement proper control measures as mentioned in the observation.
There are some small correction to be done in order eliminate and reduce the minor hazards in the
industries. Sometimes these minor hazards may become major hazards so it is important to take
actions on these minor hazards.
