ACCOMPLISHMENT REPORT

For the Period: From January to November 2018


MSWDO Project Title: Healthy Start Program

1. Capability building for Family Support Workers and Supervisor

 The Family Support Workers, parent leaders and Acting Supervisor were participated in
4th quarter activities in barangay Kilicao purok 1 to 7, Busay & Miisi Relocation site last
October 25, 30 & November 14

 Weekly documentations, regular monthly meeting was conducted every first Monday of
the month, projects update were done, issues and concerns resolved, prepared
schedule of activities and other undertaking for the month.

 Individual Supervisory (IS), shadowing and feed backing was conducted every month of
the Project Supervisor with the Family Support Workers after sessions to assess their
competency development plan.

2. Services for Family Enrolled in the Project

 There are sixty (60) families enrolled in the program who are actively participating in the
Growing Great Kids and Growing Great Families sessions and activities.

 All Partner Babies (PB) have completed their immunizations.

 Fifty-five (55) Partner Babies have normal weight; while five (5) Partner Babies have
mildly underweight. Follow-up discussions on the preparation of nutritious foods were
conducted with the mothers for the underweight babies.

 Fifty (50) had normal results in Ages & Stages Questionnaires (ASQ), five (5) delay in
gross motor & fine motor , five (5) in borderline. Proper nutrition is connected to this
because the PBs who were alert are seen to be healthy and active while the PBs in the
borderline results were less participative in the given activities. The FSWs continue to
monitor them.

 Ten (10) partner families needed medical assistance, and they were served thru
referrals.
 Forty (40) out of fifty (50) partner families who planned to finish their Individual Family
Support Plan (IFSP) were able to do as planned. Ten (10) who planned to complete their
IFSP within this year are currently working on it. They achieved their goals on their
income generating activities and other plans for the family.

 There are forty-five (45) out of sixty (60) families that are practicing family planning,
twenty-five(25) are using natural family planning, twenty (20) are using artificial family
planning method.

3. Establishment of linkages and synergy:


Identify community or area-based resources that could be accessed by
Healthy Start Families:

 DSWD/MSWDO
 RHU provided medicines, vitamins, and immunizations to the PBs and contraceptives for
the parents.
 Brgy. Health center provides first aid medicines for entire families and immunizations
for the babies.
 Government Hospital (BRTTH)
 Charity Clinic (Servant of Jesus)

Capability building for Family Support Workers and Supervisor

 The Family Support Workers was conducted their Plan of Activities last March 21, 2019
at Bichara Farm, Bagacay Legaspi City.

 The Family Support Workers visited Provincial Agriculture Office, at Cabangan, Camalig,
Albay, as part of their activities.

 Weekly documentations, regular monthly meeting was conducted every first Monday of
the month, projects update were done, issues and concerns resolved, prepared
schedule of activities and other undertaking for the month.
