EVALUATION:
During the entire conversation, the patient held smile and he is very responsive
in every questions I was asking. He said part of his life story like his work before
he was admitted, his brothers and his relatives. Somehow, he is very sad
because he said all he need is love from his family. He have this hallucinations
that there are military that are taking care of him. In addition, the patient lapses
At this point, the client’s problems are identified and solutions are
explored, applied and evaluated. The focus of the assessment and of the relationship is
the client’s behavior and the focus of the interaction is the client’s feelings.
EVALUATION:
At the end of the working phase, the patient was able to verbalize his feelings and
emotions and was able to tell his story why he was admitted to NCMH. He knows his
current situation and he feels sad for not being with his family. Yet, he still have auditory
hallucinations.

The nurse terminates the relationship when the mutually agreed goals are met,
the patient is discharged or transferred or the rotation is finished. The focus of this stage
is the growth that has occurred in the client and the nurse helps the patient to become
independent and responsible in making his own decisions. The relationship and the
growth or change that has occurred in both the nurse and the patient is summarized.
EVALUATION:
At the end of the termination phase, the patient is happy but tired due to the activities
during the socialization day. He understand that this is the last day of our interaction.
