The patient sits calmly and always smiling. He has a good eye contact and he
waggles his feet the whole time.
V. Description of the Environment
The environment was calm and therapeutic. The breeze of the wind makes the
patient feel comfortable and relaxing.

EVALUATION:
During the entire conversation, the patient held smile and he is very responsive

in every questions I was asking. He said part of his life story like his work before

he was admitted, his brothers and his relatives. Somehow, he is very sad

because he said all he need is love from his family. He have this hallucinations

that there are military that are taking care of him. In addition, the patient lapses

often on some topics which are not good in mental state.

At this point, the client’s problems are identified and solutions are
explored, applied and evaluated. The focus of the assessment and of the relationship is
the client’s behavior and the focus of the interaction is the client’s feelings.

The patient looks calm and always smiling. He leads the way on where we are
going to stay for NPI. When he sit down, he sways his feet forward and backward.

The working atmosphere was good. The wind breeze makes it more comforting
and relaxing. The patient enjoys looking at the surrounding.

EVALUATION:
At the end of the working phase, the patient was able to verbalize his feelings and
emotions and was able to tell his story why he was admitted to NCMH. He knows his
current situation and he feels sad for not being with his family. Yet, he still have auditory
hallucinations.

Termination Phase
Description of Phase:

The nurse terminates the relationship when the mutually agreed goals are met,
the patient is discharged or transferred or the rotation is finished. The focus of this stage
is the growth that has occurred in the client and the nurse helps the patient to become
independent and responsible in making his own decisions. The relationship and the
growth or change that has occurred in both the nurse and the patient is summarized.

XIV. Description of the Environment

The weather was calm and sunny. The termination phase was quite more joyous and
active since it is the last time to interact with them. Every patient cooperated and tried
their best to achieve the goals in that particular activity. At the end, they are quite sad
on that farewell but then as they verbalize that they are thankful that we had them
