PRIVACY POLICY

This Privacy Policy (“Privacy Policy”) sets out how PT Media Dokter Investama and its subsidiaries and affiliates (“We”), raise,
collect, use, display, announce, disclose, process, open access, store, send, give, assign, treats, manage, destroy and protect the
information and Personal Data (jointly, “Use”) supplied by You as user (“You”) of Halodoc Application (“Application”) or Halodoc
Web Application(“Web App”) as requested or when using the features in the Application (“Personal Data”). Please note that
Personal Data excludes Personal Data available in public domain.

This Privacy Policy is part of our Terms and conditions of Use (“Terms of Use”). Use of Platform and each feature and/or service
available in the Platform (“Service”) indicates form of your acceptance for the Terms of Use and Privacy Policy. Therefore, it is
necessary that You read this Privacy Policy thoroughly to ensure your full understanding before registering, accessing and/or
using the Application, Web App and Our Service.

A. Scope of Privacy Policy

1. Particularly for Pharmacy Delivery Feature is managed by PT MENSA MEDIKA INVESTAMA (“PT MMI”) and connected with
GO-MED feature managed by PT APLIKASI KARYA ANAK BANGSA (“GO-JEK”) in GO-JEK Application as third party delivering the
medicine, vitamin or medical equipment order.

2. This Privacy Policy governs the Use of Personal Data.

3. By using the Platform, You will be deemed to have read this Privacy Policy and agree the procedure with regard to the Use of
your Personal Data set forth in this Privacy Policy.

4. If We request You to provide information when using the Platform, such information will only be used for the purpose of
rendering the service in accordance with this Privacy Policy.

5. We shall have the right to at any time to modify, delete and apply new provisions of Privacy Policy. You are expected to check
the page of this Privacy Policy regularly to be informed of any changes. By using the Platform after such change, You will be
deemed to have acknowledged and agreed with the changes to the Privacy Policy.

B. Registration

1. You are required to make the registration in the Platform to may use of features on the Platform.

2. For registering in the Platform, You must provide the information required by us as stated in point C (Personal Data) below.

C. Personal Data

1. You acknowledge and agree that We collect personal information supplied by You when making the account and profile or
when using the features in the Platform.

2. Information on personal identity to be completed when making the account in the Platform include:

(i) Full Name as printed in the valid ID card (ID CARD or Passport)

(ii) Mobile phone number.

3. You acknowledge and agree that We also have the right to collect information when You use the Platform, including but not
limited to: GPS location/coordinate (if You activate this feature), Internet Protocol (IP) address (if using browser), operating
system, type of browser, Platform address referring to and your activity in the Platform. You can activate or deactivate the
locating service when using use the Platform.

4. You acknowledge and agree that We have the right to verify directly to You or other party of information on personal data
supplied by You through the Platform.

5. By using Platform, You represent that the information supplied is correct and accurate.

6. If the information supplied is false or incorrect, We are not responsible for all consequences in connection with the supply and
usage of false information.

D. Chat, Video and Voice Call data in Contact Doctor Feature (Contact Doctor)

For your convenience in the interaction with the doctor, chat service (You may also send the picture to doctor relating to medical
condition), video and voice call in the Platform are available. You acknowledge and agree that for the purpose of review, Video
and Voice Call Service will be automatically recorded and chat history also will be stored.

E. Responsibility for the Use of Chat, Video and Voice Call features

In using the Chat, Video and Voice Call features in the Platform, You understand that Platform is only a tool to ease the search of
Service, to give convenience and ease in the interaction with the doctor, each content and/or representations in the conversation
by You and the doctor using such features, this is a private conversation between You and the doctor and is clearly beyond our
control, all disputes relating to the content and/or representations from conversation between You and the doctor in the features,
shall be your full responsibility and You agree to release and hold us harmless from and against any and all claims, lawsuit and
legal consequences.

F. Responsibility for Storage of Laboratory Examination Medical Record in the Lab Service Feature (Laboratory Service)

a. You acknowledge and agree that You provide full right to us to treat and the data and information supplied at the time of
registration and/or ordering the lab including history and other relevant matters.

b. You acknowledge and agree that for your convenience, the result of laboratory examination will be provided in digital format
and be uploaded to your account for viewing (“Lab Examination Result”). The result of laboratory examination will be kept
confidential by the security system complying with the standard set in the applicable legislations of Indonesia.

c. You acknowledge and agree that You shall request acceptance from other persons ordering for Lab Service (Laboratory Service)
by You and be held responsible for the confidentiality of the result of medical record of the other persons in your Halodoc account.
We will be released and held harmless from and against any and all claims and suits by other persons relating to the medical
record of other persons as uploaded and stored in your Halodoc account.

G. Visitor Data

You acknowledge and agree that the Platform will collect information on use, such as (but not limited to) number of visits, source
of visitors and activity. These data will be used by us for the purpose of improving your satisfaction and our service quality.

H. Use of Personal Data

1. You hereby strictly agree with us and Service Provider to Use your Personal Data for the business, operations and other
purposes according to the intention of supply of Personal Data, including however not limited to:
(i) Provide information to You and Service Provider at your request for Our Service. We can share your health information with
other doctor or health provider to provide You with the Service through us.

(ii) Providing prescription and/or medicine at your request relating to our service with the Pharmacy Delivery feature. We can
share such information with the pharmacy or other party to provide You with the service through us.

(iii) Use and disclose your health information and prescription for the Platform operational purpose. We can use and distribute
this information for the purpose of review and improvement of service quality We provide, or competence and qualifications of
professional staff of the Platform.

(iv) Reply questions, comments and feedback.

(v) Process your participation in each contest, game, promotion, polling or survey.

(vi) Understand and analyze the use and sale of Product and service for the needs and preference.

(vii) Develop, improve and provide the Product and service to fulfill your needs.

(viii) Provide information/send e-mail and/or SMS on offer and/or promotion of new Product periodically, if there is an offer
and/or promotion on new products, special offer, or other information that You need to know according to us.

(ix) Inform You each change in the Platform, service and our Product.

(x) Use information to contact You.

(xi) Use and disclose this information if necessary for medical review, legal service and audit, including fraud and misuse detection
and adjustment programs, including business planning and management.

(xii) Our note or internal documentation stored in our server, within and outside the Republic of Indonesia.

(xiii) Provide your Personal Data to PT MMI, GO-JEK, and Service Provider for purpose of Service based on Platform.

(xiv) Other purposes to the extent permitted by law.

2. We will use the data collected for purpose of processing statistical data to be used for various medical or commercial Platform.
Processing of statistical data will not disclose individual data, however there may be possibility for disclosure of individual data
in the search of result of statistical processing. By using the Platform, You will be deemed to have understood and agreed with
the use of data.

3. We represent that data and information supplied by You are confidential and will not be disclosed except as expressed and
agreed by You in this Privacy Policy. To prevent unauthorized access, We take physical, electronic security measures and
managerial procedure to protect your information.

4. Your medical information will only be used in connection with the Platform service and matters stated and agreed by You in
this Privacy Policy and We will only collect information required to operate the Platform Service.

5. We keep your personal information as related to and for the purpose of our service to You. The confidentiality of your personal
information will be deemed confidential and will not be distributed unless agreed by You subject to this Privacy Policy or
permitted by applicable legislations.

6. By using Platform, You will be deemed to have agreed with third party to use your Personal Data to provide You information
on goods and service needed. Third party includes but not limited to:

(i) Your insurance that will need proof of service rendered and/or medical report before payment to the Platform or if required
by them for authorization of service or referral.

(ii) Third party Service Provider providing service to You or your doctor (for example x-ray or laboratory test) to receive the result
of examination or for purpose of invoicing.

(iii) Our business partner, such as pharmacy partner or collection service administrators. We have an agreement with each
business partner and Service Provider to protect the confidentiality and security of information forwarded to them.
(iv) Company research and marketing relating to promotion by us.

(v) Service provider company for information technology (IT) such as infrastructure service, cloud computing, software, analysis
development of “Big Data” and “Machine Learning”, including “Data Cleansing”, “Data Insight”, “Credit Scoring” and others.

(vi) Service provider for payment including payment gateway, transfer fund, electronic wallet, and other payment system
operator relating to the transaction in the Platform.

(vii) Other service provider or partners relating to features, Product and service offered in the Platform.

(viii) Professional advisor and external auditors, including legal advisor, financial advisor and other consultants.

(ix) Governmental authorities, both within and outside the jurisdiction of the Republic of Indonesia in accordance with applicable
legislations.

(x) Other parties to the extent permitted by applicable law.

I. Your Connection to Other Platform

The platform can contain links to third party platforms ("Third Party Platforms") and third party content ("Third Party Content").
Your use of links to Third Party Platforms and / or Third Party Content is based on your own discretion. When you use a link to a
Third Party Platform and / or Third Party Content, we are not responsible for protecting the privacy and any information you
provide when visiting Third Party Platforms and / or Third Party Content. We also are not responsible for any information
contained in Third Party Platforms and / or Third Party Content, including the effects of accessing Third Party Platforms and / or
Third Party Content. You must carefully study and read the personal information handling policies that apply to Third Party
Platforms and / or Third Party Content.

We provide links to Third Party Platforms and / or Third Party Content only for your convenience and this cannot be interpreted
as support, adoption, sponsorship, affiliation between Us and Third Party Platforms and / or Third Party Content.

J. Governing Law

This Privacy Policy shall be governed by and construed in accordance with the laws of the Republic of Indonesia.

K. Security Measures

a. We are committed to ensuring that the information supplied to us is safe and cannot be used by unauthorized parties. By using
the Platform, You allow us to take any action deemed necessary to protect your information collected online to prevent
unauthorized access. However, Please note that We cannot fully represent the security and are not responsible for the losses,
including however not limited to loss of data caused by any incident beyond our control. Therefore, We suggest You regularly
update your Platform and software and not to disclose your password to any person.

b. You hereby expressly agree and accept that We may send, store, use and treat your Personal Data in the server located at the
center approved by us. The said data center is managed by third party. However, use of information and Personal Data in
connection with use of Platform will be governed by this Privacy Policy in accordance with applicable legislations. We will keep
your personal information and Personal Data at least until specific request is made to delete your account or up to the expiration
of period in the applicable legislations.
