Dear Mr./Ms. ___________,

It has been observed that you have proceeded to leave without prior permission of the concerned
authorities during your visit for training to Hyderabad, later resigning from the job on false ground of
father’s illness, resulting in willful insubordination and gross negligence of duties.

Absenting yourself from duties without prior intimation is a misconduct and breaching the clauses of
the appointment signed by you making yourself liable for necessary action.

As per the appointment clause, either you serve notice period or pay money in lieu of notice. Since
you have not reporting to service you are expected to pay dues as per appointment clause and any
other dues to the company in the form of DD or Cheque by ______

You are further advised to submit a written explanation on your unauthorized leave or meet the
Reporting Manager/Management team within 72hours from the issue of the letter / as soon as you
receive this letter. Failure to do so within stipulated time shall invoke legal action against you and case
might be filed for all the loss company is suffering for now and future prospects.
