ASSOCIATIONS (PTA) HELD AT ____________________________ ON JULY ,2019

RESOLUTION REQUESTING THE OFFICE OF THE GOVERNOR, HONORABLE

RHODORA J. CADIAO FOR THE COMPLETION OF EVACUATION CENTER
WITH REPAIR OF STAGE IN THE AMOUNT OF TWO MILLION FIVE HUNDRED
THOUSAND PESOS (P2,500,000)

WHEREAS, the multi-purpose gym which primarily is an evacuation center is

almost complete, however, it still needs one span for its total and final completion and
the repair of the stage which is generally used for school programs, meetings and
activities;

WHEREAS, the PTA and the school have insufficient funds to implement the
construction of the completion of the evacuation center with stage at Sido San Juan
National High School;

WHEREAS, there is a need to finally finish the completion of the evacuation center
with the repair of the stage for it to be utilized and that its use be maximized as a safe
and sound edifice for the people of Sido, San Juan and nearby barangays in case there
is a calamity;
 NOW THEREFORE, BE IT RESOLVED AS IT IS HEREBY RESOLVED, by the
PTA Officials of Sido San Juan National High School in a meeting duly assembled to
request the Office of the Governor, Honorable Rhodora J. Cadiao of the Province of
Antique to allocate funds for construction of evacuation center for Sido San Juan National
High School in the amount of two million five hundred thousand pesos (P2,500,000).

RESOLVED FURTHER, that a copy of this Resolution be forwarded to the Office

of the Sangguniang Panlalawigan for indorsement to the Office of the Governor,
Honorable Rhodora J. Cadiao.

RESOLUTION REQUESTING THE OFFICE OF CONGRESSWOMAN, HON. LOREN

LEGARDA FOR THE COMPLETION OF THE MULTI-PURPOSE TEEN CENTER IN
THE AMOUNT OF ONE MILLION PESOS (P1,000,000)

WHEREAS, Sido San Juan National High School has inadequate funds to finance
the completion of the Multi-Purpose Teen Center;

WHEREAS, Sido San Juan National High School is in dire need of this Multi-
Purpose Teen Center because this will also serve as School Clinic, Office of the Supreme
Student Government (SSG) and it will likewise house our School Library;

WHEREAS, the unfinished building is not yet ready to be occupied because it was
barely finished and it remains unsafe for the teachers, faculty and most especially the
students;

NOW THEREFORE, BE IT RESOLVED AS IT IS HEREBY RESOLVED, by the

PTA Officials of Sido San Juan National High School in a meeting duly assembled to
request the Office of Congresswoman, Honorable Loren Legarda of the Lone District of
Antique to allocate funds for the completion of the Multi-Purpose Teen Center of Sido San
Juan National High School in the amount of one million pesos (P1,000,000).

RESOLVED FURTHER, that a copy of this Resolution be forwarded to the Office

of the Sangguniang Panlalawigan for indorsement to the Office of of Congresswoman,
Honorable Loren Legarda.

RESOLUTION REQUESTING THE OFFICE OF CONGRESSWOMAN, HON. LOREN

LEGARDA FOR FUNDS INTENDED FOR THE LYRE AND DRUMS CORPS FOR
THE PURCHASE OF INSTRUMENTS IN THE AMOUNT OF TWO HUNDRED
THOUSAND PESOS (P200,000).

WHEREAS, Sido San Juan National High School has scarce financial resources for
purchase of musical instruments and other necessary equipment such as uniforms for its
lyre and drums corps;

WHEREAS, Sido San Juan National High School lyre and drums corps often
participates in various programs, activities and parades in the barangay, municipal and
district level;
 WHEREAS, Sido San Juan National High School lyre and drums corps is a helpful
organization for its students to develop and hone its skills and talents most especially in
the field of music;

NOW THEREFORE, BE IT RESOLVED AS IT IS HEREBY RESOLVED, by the

PTA Officials of Sido San Juan National High School in a meeting duly assembled to
request the Office of Congresswoman, Honorable Loren Legarda of the Lone District of
Antique for funds intended for the purchase of instruments of Sido San Juan National
High School lyre and drum corps in the amount of two hundred thousand pesos
(P200,000).

RESOLVED FURTHER, that a copy of this Resolution be forwarded to the Office

of the Sangguniang Panlalawigan for indorsement to the Office of of Congresswoman,
Honorable Loren Legarda.
