Safety Alert - Collapse of Falsework
Incident
On 25th June 2019, a falsework collapsed during concreting work in construction site in Kuala Lumpur. A corridor
of one level gave way. It affected three levels of corridors down below.

Investigation
Based on the investigation, cause of the incident was the falsework structure not capable to bear the applied
load. Lack of understanding on structural shoring details and requirements could possibly be the
underlying cause of the incident.
Conclusion/Actions
a) The formwork and reshores shall be certified structurally safe by a Professional Engineer.
b) A designated person shall supervise the erection of the formwork including the shores, braces and other
supports.
c) Upon erection of formwork, the designated person shall make a thorough inspection to ensure that the
formwork is safe.
d) A designated person shall regularly inspect the formwork, shores, braces and other supports during the
placing of concrete. Reshores shall be similarly inspected.
e) Any unsafe condition discovered during the inspection mentioned in sub-regulations (2) and (3) shall be
remedied immediately.
f) The designated person shall keep all records of such inspections at the worksite and shall produce them for
examination at the request of the DOSH.
