OBJECTIVES

General Objective

At the end of this case, the proponent will be able to acquire and put into practice all the
essential knowledge, skills and attitudes regarding Hypertensive Intracerebral Hemorrhage and
the necessary nursing considerations that should be performed.

Specific Objectives

1. To identify the patient’s profile in order to address correct interventions and medications.

2. To analyze the health history of the patient in order to correlate the past health history as well
as the family, personal, social, psychological history of the patient and the history of present
illness to arrive to a definitive diagnosis.

3. To assess the patient from head-to-toe in order to analyze the health status of the patient as
well as the abnormalities present.

4. To analyze and interpret the laboratory examinations in order to assess the client's health and
to determine whether the diagnosis could be supported by the laboratory results.

5. To analyze the anatomical parts involved in the patient's diagnosis in order to identify the
malfunctioning organ and how it function normally.

6. To analyze the disease through clinical discussions and course in the ward in order to
determine the etiology, signs and symptoms and complications of the disease as well as its
progression.

7. To formulate and utilize priority Nursing Care Plan in order to provide a necessary action to
prevent the possible progression of the disease.

8. To analyze the drug study in order to be familiarized with the correct medications for the
disease and to provide an appropriate nursing considerations and monitoring parameters to
prevent drug intoxication.

9. To formulate a discharge planning in order to educate the family of the patient for the most
appropriate action to be done at home in enhancing the client's health and emphasize importance
of follow-up check up to watch out for untoward sign and symptoms.
