(Resolution for the Collection of Club Membership Fee)

To all Gentlemen/Ladies, Parents and Teachers:

Excerpts from the minutes of the General/ Homeroom PTA Meeting of _________ National High School, Junior
and Senior High School P.T.A. and teachers meeting last June 3, 2018 at ______________National High School,
Himamaylan City Negros Occidental.

WHEREAS, the parents and teachers understand the need of raising funds for some specific purpose to
augment and finance certain programs.

WHEREAS, the parents and teachers requested the approval of the Principal of __________ National High
School to raise such fund however be collected as donations from the parents beginning June 2018 to March
2019.

WHEREAS, the officers and members of the PTA of Aguisan National High School voluntarily agreed to
contribute Three Hundred Fifty Pesos(P 350.00 ) per student for school year 2018- 2019.

WHEREAS, the officers and members of the PTA of Aguisan National High School authorize _______________ ,
SSG Treasurer, to collect the said amount every grading period and to be supervised by the SSG Adviser
onwards.

WHEREAS, this resolution will be disseminated to all students and their respective parents, guardians and
teachers.

NOW THEREFORE, a motion of ______________________________________________, duly seconded by

____________________________________ and all its officers and members of ANHS PTA it is…

RESSOLVED and SIGNED this 3rd day of June 2018 at Aguisan , Himamaylan City, Negros Occidental.

Excerpts from the minutes of the General/ Homeroom PTA Meeting of _________ National High
School, Junior and Senior High School P.T.A. and teachers meeting last June 14, 2019, ( 1st General
PTA Meeting) at __________ National High School, Himamaylan City, Negros Occidental.

WHEREAS, the parents and teachers understand the need of raising funds for some specific purposes
to augment and finance certain programs.

WHEREAS, the parents and teachers requested the approval of the Principal of Aguisan National High
School to raise such fund however be collected as donations from the parents beginning July 2019 to
March 2020, membership fee included: Mapeh -20, Filipino- 10, Science -10, Math -10, English-10,
Yes-O -10, School Pub-60, Values-10 , TLE-10 .

WHEREAS, the SSG Club membership 60.00 ( AP 10 ), AID-10 and Guard- 60 , will be collected by the
treasurer of SSG Club and supervised by SSG adviser onwards, and STEP-40, will be collected by over-
all STEP Treasurer.

WHEREAS, the officers and members of the PTA of Aguisan National High School voluntarily agreed to
contribute Three Hundred Fifty Pesos(P 330.00 ) per student for school year 2019- 2020.

WHEREAS, the officers and members of the PTA of Aguisan National High School authorize PTA
Federated Treasurer ____________________________________, to collect over-all the said amount
every grading period and end of school year.

WHEREAS, this resolution will be disseminated to all students and their respective parents, guardians
and teachers.

NOW THEREFORE, a motion of ______________________________________________, duly
